摘 要:目的了解Ⅰ、Ⅱ类切口手术部位医院感染现状,为进一步干预提供依据。方法采取前瞻性调查与回顾性调查相结合方式,对医院2009年15个临床手术科室住院患者Ⅰ、Ⅱ类切口手术部位发生医院感染情况进行分析。结果共计监测手术病例8599例,发生手术部位相关性医院感染共44例,感染率为0.51%;其中,Ⅰ类切口手术4717例,发生医院感染4例,感染率为0.08%,Ⅱ类切口手术3882例,发生医院感染40例,感染率为1.03%;按不同手术危险指数级别统计:危险指数评分为0分病例共2984例,未发生手术部位感染病例,危险指数评分为1分病例共4053例,发生手术部位感染22例,感染率为0.54%,危险指数评分为2分病例共1485例,发生手术部位感染20例,感染率为1.35%,危险指数评分为3分病例共77例,发生手术部位感染2例,感染率为2.60%。结论随着手术切口污染程度的增加和危险指数(包含手术时间、切口类型和ASA患者病情评估)评分的增高,医院感染发生率与其成正比。OBJECTIVE To explore the nosocomial infection status of Ⅰ,Ⅱ Class surgical incision site in the hospital to provide bases for further intervention.METHODS Prospective study and retrospective investigation were taken to research Ⅰ,Ⅱ Class surgical incision site of 15 departments of clinical surgery in hospital in 2009.RESULTS Totally 44 cases occurred nosocomial infection in 8599 cases.The nosocomial infection rate was 0.51%.4 cases occurred to nosocomial infection in 4717 cases of Ⅰ type incision.The infection rate was 0.08%.40 cases occurred nosocomial infectional in 3882 cases of Ⅱ type incision.The infection rate was 1.03%.According to different levels of surgical risk index statistics,there were 2984 cases of 1 score risk index without surgical site infection occurred.22 cases occurred to nosocomial infection in 4053 cases of 2 score risk index.The infection rate was 1.35%.2 cases occurred nosocomial infectional in 77 cases of 3 score risk index.The infection rate was 2.6%.CONCLUSION With the increasing of incision pollution levels and risk index score(including operative time,incision type,and condition assessment of patients ASA),the incidence of nosocomial infection increases.
