摘  要:目的观察短期持续皮下胰岛素输注(CSII)治疗对初诊2型糖尿病(T2DM)患者胰岛素抵抗和氧化应激状况的影响。方法 60例初诊T2DM患者接受为期2周的CSII强化治疗,于治疗前、后测定血清活性氧活力(ROS)、丙二醛(MDA)、谷胱甘肽(GSH)和超氧化物歧化酶(SOD)的水平,同时测定空腹胰岛素水平,采用稳态模型计算胰岛素抵抗指数(HOMA-IR)。结果①T2DM患者的ROS和MDA明显增高,GSH和SOD明显降低。②ROS、MDA与HOMA-IR呈正相关,GSH、SOD与HOMA-IR呈负相关。③CSII治疗2周后,ROS、MDA明显降低,GSH、SOD明显增高,同时HOMA-IR亦明显降低。结论 T2DM患者体内氧化应激明显增强,与胰岛素抵抗形成有关,短期CSII除迅速降低血糖外,还具有降低T2DM患者氧化应激水平,减轻胰岛素抵抗。【Objective】 To observe the influence of short-term continuous subcutaneous insulin infusion(CSII) therapy on insulin resistance and conditions of oxidative stress in newly diagnosed type 2 diabetes(T2DM) patients.【Methods】 60 cases of newly diagnosed T2DM patients received CSII therapy for two weeks.Serum levels of reactive oxygen species(ROS),malondialdehyde(MDA),glutathione(GSH),superoxide dismutase(SOD) were examined pre and post the treatment.And the insulin resistance index(HOMA-IR) of patients with T2DM was determined using steady-state model.【Results】 ①T2DM patients' ROS,MDA were significantly increased,GSH,SOD significantly decreased.②ROS,MDA were positive correlation with HOMA-IR,GSH,SOD were negative correlation with HOMA-IR.③After CSII treatment for 2 weeks,ROS,MDA decreased,GSH,SOD significantly increased,while HOMA-IR also decreased significantly.【Conclusion】 Oxidative stress in T2DM patients increased,and related to insulin resistance.In addition,short-term CSII can lower blood sugar quickly,but also lower levels of oxidative stress in patients with T2DM,and therefore reduce insulin resistance.
