摘  要:目的研究妊娠早期妇女血清促甲状腺激素(TSH)和血压的关系。方法选取2008年8月至2010年3月在江苏省丰县妇幼保健院进行早孕确认的妊娠早期妇女305例,填写问卷,测量血压、身高、体重,空腹抽血测血清TSH,TSH异常者进一步测血清游离三碘甲状腺原氨酸(FL)和游离甲状腺素(FT4),分析不同甲状腺功能状态组的收缩压(SBP)和舒张压(DBP)的特征,血清TSH与收缩压、舒张压的关系,比较各组高血压以及高血压前期的患病率和患病的危险性。结果(1)亚临床甲状腺功能减退(亚临床甲减)组与甲状腺功能正常组(甲功正常组)相比,SBP、DBP差异无统计学意义。(2)不同血清TSH与收缩压和舒张压均无关。(3)亚临床甲减组高血压前期及高血压的总患病率高于甲功正常组(36.1%对29.8%),但差异无统计学意义(P〉0.05)。经校正年龄、BMI后,妊娠早期亚临床甲减与高血压及高血压前期无独立相关(OR=1.399,95%CI 0.778~2.516,P=0.262)。结论妊娠早期TSH与孕妇血压无关,妊娠早期亚临床甲减与高血压及高血压前期无独立相关。Objective To investigate the relationship between serum level of thyroid stimulating hormone (TSH) and blood pressure during early trimester of pregnancy. Methods 305 pregnant women in first trimester of pregnancy confirmed at Women's and Children's Health Hospital of Fengxian,Jiangsu Province,between August 2008 and March 2010 were recruited in the study. The subjects were instructed to complete a questionnaire, meas- ured for blood pressure, height and body weight, and collected fasting blood samples for determination of TSH. FT3 and FT4 were further examined if TSH was abnormal. The correlations between levels of serum TSH and systolic and diastolic blood pressure ( SBP and DBP) were analyzed. Results ( 1 ) SBP and DBP in patients with subclin- ical hypothyroidism were comparable with those in normal subjects. (2)Different levels of serum TSH were not correlated with SBP and DBP. ( 3 ) The overall prevalence of hypertension/prehypertension in patients with subclinical hypothyroidism was higher than in euthyroid group (36. 1% vs 29. 8% ), but the difference did not reach level of statistical significance (P 〉 0. 05 ). After adjustment for age and body mass index (BMI), subclinical hypothyroidism during the first trimester of pregnancy was not found to significantly correlate with hypertension/prehypertension (OR = 1.399,95%CI 0.778 -2.516 P =0. 262). Conclusion The levels of serum TSH are not linked to blood pressure in women with early pregnancy. Subclinical hypothyroidism during the first trimester is not independently correlated with hypertension/prehypertension.
