摘　　要：目的了解浙江省麻风留院者足底溃疡经济负担情况,为麻风足底溃疡防治工作提供理论依据。方法选取浙江省集中收治的3个麻风村,采用自制的麻风足底溃疡经济负担调查表,由经培训的麻风防治人员对每个有足底溃疡的患者进行调查。结果 3个麻风村共调查足底溃疡病人81人,其中有70例足底溃疡为复杂性溃疡,11例为单纯性足底溃疡。近一年因足底溃疡所用直接经济负担：换药费为400~7 300元,总计250 003.80元,平均为3 378.43元;治疗费为100~3 800元（包括6例溃疡截肢患者）,总计31 530.70元,平均为389.27元;药费为90~3 000元不等,总共为30 377.40元,平均为506.29元;检查费为4 072.50元,平均为50.28元。间接经济负担：营养费为20076元,平均为247.85元;休息天数为5~365天,总共5 041天,平均为62.23天;照顾费为25 146.5元;防护鞋费用为50~150元,总计5605.7元,平均为69.21元。结论足底溃疡是麻风病人常见的并发症,其经济负担比较重,国家应加大这方面的卫生资源投入,减轻麻风留院患者的经济负担。Objective To understand financial burden of leprosy hospital patients with plantar ulcer in Zhejiang province and provide a theoretical basis for preventing leprosy＇s plantar ulcer.Methods The three leprosy villages were selected as the survey respondents by using self-made questionnaire for economic burden of leprosy plantar ulcer by trained personnel on prevention and treatment of leprosy plantar ulcer patients.Results There were eighty three leprosy patients with plantar ulcers in the villages,including 70 of the patients with complicated plantar ulcer and 11 of the patients with simple foot ulcers.The direct economic burden by foot ulcers in the past year including： dressing fee is 400-7300 yuan,a total of 250 003.80 yuan,an average of 3 378.43 yuan;treatment fee is 100-3 800 yuan（including six cases of amputation ulcer patients）,a total of them is 31 530.70 yuan,with average of 389.27 yuan;medicine fee is 90-3 000 per month,a total of 30 377.40 yuan,an average of 506.29 yuan;inspection fee is 4 072.50 yuan,an average of 50.28 yuan.Indirect economic burden： nutrition fee is 20 076 yuan,an average of 247.85 yuan;protective footwear expenditure is 50-150 yuan,a total of 5,605.7 yuan,an average of 69.21 yuan.Conclusion Foot ulcers are a common complication of leprosy with relatively heavy economic burden.The state should increase investment in this area of health resources to reduce the leprosy burden in hospital patients.
