静脉推注蔗糖铁治疗肾性贫血临床分析

摘　　要：目的:对比观察静脉推注蔗糖铁、静脉推注右旋糖酐铁、口服多糖铁复合物胶囊(力蜚能)治疗肾性贫血的疗效及不良反应。方法:选取收治的终末期肾病血液透析中重度肾性贫血(血色素<90 g/L)患者63例,随机分为三组各21例,三组分别给予静脉推注蔗糖铁、静脉推注右旋糖酐铁、口服多糖铁复合物胶囊(力蜚能)治疗,三组均同时联合使用重组人红细胞生成素(rHuEPO)治疗,共治疗12周,观察血色素(Hb)、红细胞比容(Hct)、血清铁蛋白(SF)、转铁蛋白饱和度(TS)、肝功等指标及药物不良反应。结果:蔗糖铁组总有效率为90.47%,右旋糖酐铁组总有效率为84.21%,力蜚能组为71.43%,三组比较,蔗糖铁组与右旋糖酐铁组疗效比较,差异无统计学意义(P>0.05),但前两组与力蜚能组疗效比较,差异有统计学意义(P<0.05);蔗糖铁组明显药物不良反应发生率为4.76%,力蜚能组发生率为4.76%,而右旋糖酐铁组发生率为19.04%,前两组与右旋糖酐铁组比较,差异有统计学意义(P<0.05)。结论:静脉推注蔗糖铁联合rHuEPO治疗肾性贫血疗效较好,且不良反应少,可作为首选方法之一。Objective To compare intravenous iron sucrose injection,intravenous injection of iron dextran,oral iron complex polysaccharide capsule（force enjoys a can）in the treatment of renal anemia and side effects.Methods in our hospital hemodialysis in end-stage renal disease and severe renal anemia（hemoglobin90 g/L）in 63 patients were randomly divided into three groups of 21 patients,three were given intravenous iron sucrose injection,intravenous injection of iron dextran,oral iron complex polysaccharide capsule（force enjoys energy）treatment,three groups were also combined use of recombinant human erythropoietin（rHuEPO）therapy,were treated for 12 weeks,and hemoglobin（Hb）,red blood cells hematocrit（Hct）,serum ferritin（SF）,transferrin saturation（TS）,and other indicators of liver function and drug side effects.Results The total effective rate of iron sucrose 90.47%,iron dextran total effective rate was 84.21%,Li Fei can group was 71.43% in the third group,the iron sucrose group and the efficacy of iron dextran group was no significant difference（P0.05）,But the ？rst two groups compared with the power to effect enjoys a significant difference（P0.05）;iron sucrose group was 4.76% incidence of side effects,ability to group enjoys incidence of 4.76%,while the incidence of iron dextran group was 19.04%,the first two groups compared with iron dextran significantly different（P0.05）.Conclusion rHuEPO intravenous iron sucrose combined effect of better treatment of renal anemia,and less side effects and can be used as the preferred methods.
