摘　　要：目的:研究多囊卵巢综合征(PCOS)痰湿证患者卵巢黄素化颗粒细胞胰岛素受体底物(IRS)1/2mRNA的表达,探讨其代谢及生殖功能紊乱的发病机制。方法:采用化学发光法检测PCOS痰湿组、PCOS非痰湿组及对照组血清空腹胰岛素(FINS)、空腹血糖(FPG)的浓度;稳态模型计算胰岛素抵抗指数(HOMA-IR);RT-PCR方法检测IRS1/2mRNA的表达。结果:①PCOS痰湿组FINS及HOMA-IR均显著高于PCOS非痰湿组与对照组(P<0.05);②PCOS痰湿组卵巢颗粒细胞IRS-1mRNA的表达明显高于对照组及非痰湿组(P<0.05);PCOS非痰湿组IRS-1mRNA的表达水平也明显高于对照组;③PCOS痰湿组IRS-2 mRNA的表达明显低于对照组及非痰湿组(P<0.05)。结论:PCOS痰湿证与胰岛素抵抗有明显的相关性,其卵巢局部IRS1/2mRNA的表达失衡可能是其产生胰岛素抵抗的发病机制之一。Objective：To investigate mRNA expression of insulin receptor substrate1/2（IRS1/2） in luteinizing granulosa cells of polycystic ovary syndrome（PCOS） patients with phlegm-dampness,and explore molecular mechanisms of insulin resistance（IR） in PCOS with phlegm-dampness.Methods：Samples of PCOS patients with phlegm-dampness（n=20）,PCOS patients without phlegm-dampness（n=20） and the controls（n=25） were taken.Fasting serum insulin（FIN） and fasting plasma glucose（FPG） were examined by using chemiluminescence.Insulin resistance index（IRI） was calculated using homeostasismodel assessment（HOMA）.The expression of IRS 1/2 in luteinizing granulosa cells was detected by using RT-PCR.Results：①The levels of serum FIN and Homa-IR of PCOS women with phlegm-dampness were significantly higher than those of the PCOS women without phlegm-dampness group and the control group（P0.05）;②The expression of IRS-1 mRNA increased significantly in the PCOS women with phlegm-dampness as compared with that of the PCOS women without phlegm-dampness and the controls（P0.05）;The level of IRS-1 mRNA of the women with PCOS without phlegm-dampness was significantly higher than that of women in control group;③The expression of IRS-2 mRNA decreased significantly in the PCOS women with phlegm-dampness as compared with that of the PCOS women without phlegm-dampness and the controls（P0.05）.Conclusion：There was significant relation between PCOS phlegm-dampness and insulin resistance,IRS-1/2 ratio imbalance may be the main mechanism of IR in phlegm-dampness PCOS patients.
