摘 要:目的:研究面神经低位切断伤后,应用重组NTN(Neurtuin)神经营养因子/PGLA导管修复,对兔面运动神经元的保护作用。方法:手术制作新西兰兔左侧面神经低位切断伤+自体神经修复模型,右侧低位切断伤+NTN/PGLA修复模型,运用辣根过氧化物酶逆行追踪标记,甲苯胺蓝染色对面运动神经元的分布、数量和形态进行定性、定量观察。结果:术后早期,NTN/PGLA的修复有效改善细胞形态,伤后5周存活神经元细记数明显比自体神经移植多(P<0.05);伤后10、14周,NTN/PGLA的保护作用减弱,NTN/PGLA侧与自体神经移植侧存活神经元记数无显著性差异,HRP逆行追踪NTN/PGLA侧于术后10周成功标记到FMN,且两侧标记的细胞数无明显差异。术后14周自体神经移植侧标记细胞出现神经元的分布异位,而NTN/PGLA侧仅1例出现。结论:成年动物(兔)面神经低位切断伤后引起面运动神经元死亡其形式以凋亡为主,兼有坏死;应用NTN/PGLA导管的修复早期对FMNs有明显保护作用,长期则可达到与自体神经移植修复相当的效果;且神经元分布异位明显少于自体神经移植修复。Objective: To elucidate the role of recombinant PGLA(poly glycolic co-lactic acid) tube containing neurturin(NTN) in the protection of facial motoneurons after facial nerve transection.Methods: Left facial nerve of rabbit was performed distal transection plus Neutrin and PGLA local application.Right facial nerve was acted as distal transection and nerve autograft.By light microscope and HRP stain,we quantitativaly and qualitativly observed the facial motoneurons follwing facial nerve transection.Results: All animals' FMNs appeared apoptosis after injury.At the 5 weeks after injury,the numbers of suvival neurons in the NTN/PGLA group were more than those in the autograft group(P0.05).At 10 and 14 weeks after the injury,the protection of NTN/PGLA decreased obviously,and the number of suvival neurons of the NTN/PGLA side had significant difference to that of 5 weeks after injury(P0.05).At 10 and 14 weeks after injury,the number of suvival neurons in the NTN/PGLA group was not significant differeny to that in the autograft group.HRP labelled neurons were found in the NTN/PGLA group at 10 weeks after injury,which indicated the axoplasm flow of regenerated nerve fiber was recovered.The number of the labelled neurons was not significant different between the NTN/PGLA group and the autograft group.At 14 weeks after injury the reorganization of labelled neurons'distribution was found in the autograft group,while only a case in the NTN/PGLA group.Conclusion: Distal transection of adult rabbit's facial nerve results to death of the facial motoneurons.In the early stage of facial nerve injury,NTN/PGLA improves the neuron's ultramicrostructure and decreases the cell death.The long-term effect might be compared with the nerve autograft.
