G-CSF对心肌梗死合并心力衰竭患者外周血内皮祖细胞和炎性因子的影响

摘　　要：目的观察应用粒-细胞集落刺激因子(G-CSF)动员内皮祖细胞(EPCs)治疗心肌梗死合并心衰患者合并外周血内皮祖细胞(EPCs)和炎性因子的变化,探讨动员剂G-CSF对外周血及炎性因子的影响。方法 38例心肌梗死合并心衰患者被随机分为G-CSF(600μg/d,共7 d)组和对照组,12例健康者为空白组。分离、培养EPCs并测定其数量。应用酶联免疫法及放射免疫法检测两组患者及10例健康者血C-反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、基质细胞衍生因子-1(stromal cell-derived factor-1,SDF-1)的水平。结果心肌梗死合并心衰患者EPCs数量和血CRP、IL-6、TNF-α及SDF-1水平均明显高于健康者(P<0.05);两组治疗后各组与治疗前相比差异有统计学意义(P<0.05);治疗组CRP水平较对照组明显降低(P<0.01);治疗组血浆SDF-1水平较对照组明显升高(P<0.01),SDF-1浓度与循环EPCs数量呈正相关;治疗组与对照组相比,IL-6、TNF-α虽有所下降,但差异无统计学意义(P>0.05)。结论 G-CSF可有效动员急性心肌梗死患者外周血EPCs,亦可降低CRP、改变SDF-1的水平。Objective To investigate the effects of granulocyte colony-stimulating factor（G-CSF） on endothelial progenitor cells（EPC） and inflammatory factors in patients with myocardial infarction（MI） and heart failure（HF）.Methods Thirty-eight patients with MI and HF were randomly divided into G-CSF group and control group,while 12 healthy people were enrolled as health group.The absolute count of CD34 positive cells in peripheral blood was enumerated with flow cytometry.Plasma levels of C-reactive protein（CRP）,inter leukin-6（IL-6）,tumor necrotic factor-α（TNF-α）,and stromal cell-derivedfactor-1（SDF-1） were measured with enzyme-linked immunoassay and radioimmunoassay.Results Peripheral endothelial progenitor cells significantly increased shortly after using G-CSF and peaked on day 7th compared with those of controls（P0.05）.The levels of CRP,IL-6 and TNF-α in patients with MI were significantly higher than those in the healthy controls（P0.05）.The plasma level of CRP in G-CSF group began to increase from day 3,but only showed significantly higher levels than that in controls（P0.05） on day 10.However,no significant difference was revealed in IL-6 and TNF-α between G-CSF and control groups.Conclusion G-CSF effectively mobilizes the endothelial progenitor cells and increases the level of plasma CRP in patients with MI.
