小儿大肠埃希菌肺炎的抗生素治疗

摘　　要：目的:对小儿大肠埃希菌肺炎的抗生素治疗结果进行研究。方法:取住院的小儿社区获得性肺炎(CAP)患儿呼吸道分泌物进行细菌培养和鉴定,明确为大肠埃希菌感染的患儿给予抗生素治疗,对患儿病历资料进行回顾性分析。结果:CAP患儿呼吸道分泌物大肠埃希菌的检出率为4.7%(55/1 167),感染病例以1岁以下小婴儿为主。大肠埃希菌与其亚种细菌感染的CAP患儿抗生素治愈率比较差异无统计学意义(χ2=0.6465,P>0.05),但产酶率差异有统计学意义(χ2=8.7949,P<0.01)。产酶与非产酶大肠埃希菌感染患儿的抗生素治愈率差异无统计学意义(χ2=0.3792,P>0.05)。头孢米诺对感染大肠埃希菌CAP患儿的治愈率与其它第三代头孢菌素的治愈率相比,差异有统计学意义(χ2=74.1522,P=0.00)。结论:抗生素治愈率与大肠埃希菌产酶(ESBLs)与否无关;头孢米诺对大肠埃希菌感染、尤其是产ESBLs的菌株感染,与其它第三代头孢菌素或第二代头孢菌素相比,具有较高的敏感性。Objective： To study the outcome of antibiotic treatment of pneumonia infected with Escherichia coli in children.Methods：Respiratory secretion specimens were collected in 1,167 children with CAP,and the specimens were used to culture bacteria.The 55 pediatric patients who were infected with Escherichia coli（E.coli） accepted antibiotic treatment respectively.Then retrospective analysis of medical records of hospitalized children was performed.Results：The isolated rate of E.coli was accounted for 4.7%（55/1167）,the majority of infected cases were infants under the age of one.As to the cure rates by antibiotics,there was no significant difference between the cases infected with E.coli and inactive form of E.coli（χ2=0.6465,P0.05）.While,to the ESBLs producing rates,there was significant difference between E.coli and an inactive form of E.coli（χ2=8.7949,P0.05）.The cure rates by antibiotics were not significantly different between the cases infected with ESBLs producing and non-producing E.coli（χ2=0.3792,P0.05）.The cure rates by antibiotics had significant difference between those with cefminox and those with the other third generation cephalosporins（χ2=74.1522,P=0.00）.Conclusions：The cure rates by antibiotics is not related to ESBLs producing or not of E.coli;the clinical sensitivity of cefminox to the ESBLs producing E.coli is higher than that of the third and the second generation cephalosporins.
