摘 要:目的比较3种固定材料治疗掌指骨骨折的临床效果。方法连续入选我科经手术治疗的掌指骨骨折患者129例(158处),随机分为AO微型钢板内固定组、微型外固定架组和克氏针内固定组,术后随访6~12个月,平均(8.2±1.8)个月。按照TAFS的评级标准对各组指间及掌指关节功能、感染情况和骨折愈合等情况进行评定。结果 AO微型钢板内固定组优良率为90.7%,外固定组优良率为88.4%,克氏针内固定组为51.2%,前两组间优良率无明显差异,但高于克氏针内固定组(P<0.01)。结论 AO微型钢板内固定或微型外固定架治疗掌指骨骨折疗效优于克氏针内固定,有利于患者早期功能锻炼和康复。Objective To compare the clinical fixed efficacy of AO miniplates,small fragment external fixation skeleton and K-wire on metacarpophalangeal fractures.Methods 129 cases of 158 metacarpal and/or phalangeal fractures were randomly divided into three groups.They were treated with AO miniplatess,small fragment external fixation skeleton and K-wire,respectively.The duration of postoperative follow-up ranged from 6 to 12 months with(8.2±1.8)months on average.Bone healing status,the functions of metacarpophalangeal and interphalangeal joints,and infection status were evaluated according TAF scale.Results The excellent and good rate of 90.7 % and 88.4% was achieved in cases treated with AO miniplates and small fragment external fixation skeleton respectively,and in those fixed with K-wire it was 51.2%.There was no significant difference between the two former groups although both of them showed significant higher excellent and good rate than K-wire group(P0.01).Conclusion Fixation by AO miniplates and small fragment external fixation skeleton can obtain better therapeutic effects on metacarpal and/or phalangeal fractures than that by K-wire.It is useful for the early functional exercise and rehabilitation by using these two methods.
