评估心肌酶与肌钙蛋白I对儿童心肌损伤的诊断价值

摘　　要：目的评估血清心肌酶与肌钙蛋白I对心肌损伤的诊断价值。方法分析及对比腹泻病患儿的血清心肌酶及肌钙蛋白1的异常值。结果血清心肌酶及心肌酶肌酸激酶同工酶(CK-MB)增高与肌钙蛋白I改变不平行,CTnI升高并不完全随心肌酶的升高而升高。结论心肌酶升高不能完全反映心肌损伤,心肌酶除CK-MB外天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)、羟丁酸脱氢酶(HBDH)、肌酸激酶(CK)不是心肌细胞特有的酶,特异性较差,心肌酶受年龄、时效、溶血等因素影响较大。对于发病时间较长者,直接检查CTnI意义重大,必要时联合检测血清CTnI及心肌酶可提高心肌损伤诊断的阳性率和准确性。Objective To assess the value diagnosis of enzyme and troponin I in serum of myocardial damage. Methods To compare the outliers of infant diarrhea on serum enzymes and troponin I. Results Serum enzymes and enzyme creatine kinase（ CK -MB）and troponin I changes were not increased in parallel, CtnI increase was not entirely the temperature increased with the enzyme. Conclusion Enzyme increases was not fully reflect myocardial injury. In addition to CK - MB external enzymes aspartate aminotransferase（AST） , lactate dehydrogenase （LDH） , hydroxybutyrate dehydrogenase （HBDH） , creatine kinase （CK） enzyme was not specific to myocardial cells, Poor specificity,enzyme affected by age, aging,and other factors hemolytic large. The time of onset was longer, direct reference checks CtnI large, if necessary, the United serum CtnI and cardiac enzymes can improve the positive rate of myocardial damage and accuracy of diagnosis.
