单孔腹腔镜经脐“Z”形切口逆行切除小儿阑尾

摘　　要：目的总结经脐“Z”形切口单孔腹腔镜逆行切除小儿阑尾的临床体会。方法回顾性分析16例自脐“Z”形切口置单孔腹腔镜将阑尾拖出体外进行逆行方法切除患儿的病例资料。结果16例中11例行单孔法腹腔镜逆行阑尾切除术获得成功，3例改为两孔法，2例中转开腹，5例术后脐部感染。结论经脐“Z”形切口单孔腹腔镜逆行阑尾切除安全可行，具有微创化、创伤小、痛苦少、康复快、隐蔽性及美容效果好等特点。Objective To summarize the experience of one -pore through the umbilical ＂Z＂ incision in laparoscopic retrograde appendectomy. Methods Laparoscope introduced into abdominal cavity through the single umbilical incision, and fininshed the retrograde appendectomy outside of abdomen. Resuits Eleven cases were operated successfully, 2 cases were coverted to two -pots, 1 case was converted to laparotomy, 5 cases had umbilical infection postoperative. Conclusions One -pore pull -through laparoscopic retrograde appendectomy is safe and feasible for children, and is really microinvasive, reflected the characteristic of less wound, less pain, faster rescovery, good concealment and cosmetic results.
