瑞芬太尼复合丙泊酚用于腹腔镜妇科手术的麻醉效果

摘　　要：目的比较瑞芬太尼或芬太尼复合丙泊酚用于腹腔镜妇科手术麻醉的临床效果。方法将60例腹腔镜妇科手术患者随机分为观察组（R组）、对照组（F组）各30例。R组持续静脉泵入丙泊酚和瑞芬太尼，F组持续静脉泵入丙泊酚和芬太尼。观察比较患者麻醉诱导前（T1）、诱导后（T2）、插管后（T3）、充气后30S（T4）、清醒时（T5）各时点血压、心率；记录术毕停药后患者自主呼吸恢复时间、呼之睁眼时间、拔管时间、定向力恢复时间。结果R组T2、T3、T4时的动脉压均高于观察组F组（P〈0．05），F组呓时的HR高于R组（P〈0．05），而T5时的HR低于R组（P〈0．05），两组自主呼吸恢复时间、呼之睁眼时间、拔管时间、定向力恢复时间比较差异均有统计学意义（P〈0．05）。结论与芬太尼复合丙泊酚麻醉下行腹腔镜妇科手术比较，瑞芬太尼复合丙泊酚能使围术期麻醉更平稳，是一种安全、有效的麻醉方法。Objective To compare the anesthetic effect of remifentanil and fentanyl combined with propofol in laparoscopic gynecological surgery. Methods Randomly divide 60 patients for laparoscopic gynecological surgery into two groups as observation group （group R） and control group （group F）. Continuously pump propofol and remifentanil into the veins of patients in group R, whereas use propofol and fentanyl to group F. Observe the blood pressure and heart rate at the following time points, before the induction of anesthesia （ T1 ） , after the induction （ T2 ） , after intubation （ T3 ） , after thirty seconds of filling （T4） and awake time （ T5 ） , record the recovery time for spontaneous breathing, respiratory, eye opening time after waken up, extubation time and recovery time for orientation. Results The arterial pressure of group R was higher than group F （P 〈 0.05 ） at the time points of T2, T3, T4, and the HR of group R was lower than group F （P 〈 0.05） at the time point of T5. Meanwhile, the recovery time for spontaneous breathing, eye opening time after waken up and recovery time for orientation have significant differences （P 〈 0.05 ）. Conclusions Comparing with fentanyl combined with propofol, remifentanil combined propofol is more stable in perioperative anesthesia in laparoscopic gynecologic surgery.
