摘　　要：目前：探讨自制带阀门血管内支架堵闭装置在巨大动脉导管未闭（ＰＤＡ） 动物模型及临床病例的应用效果。方法：对１０ 只巨大ＰＤＡ 实验模型动物，经股动脉插入血管内支架堵闭装置，完成堵闭ＰＤＡ。６ 例ＰＤＡ 病人，经８ ～１４Ｆ血管鞘通过股动脉或股静脉，顺利送入堵闭装置，操作一次成功。结果：所有的实验动物堵闭均一次成功，于术后１～１８０ 天解剖动物，行病理检查见ＰＤＡ 堵闭完全，达到永久性闭合。临床病例术毕即刻动脉造影，未见残余分流；随访１５ ～３６ 个月，恢复良好。结论：本组动物实验及临床的成功应用提示，该装置对巨大ＰＤＡ 及６ 岁以下的婴幼儿ＰＤＡ 的堵闭效果肯定，并可望堵闭内径在１２ ～２０ｍｍ 的巨大ＰＤＡ。Aim To elucidate the experimental and clinical effect of self-made stent with valve on occlusion of the large patent ductus arteriosus (PDA).Methods:Ten models of large PDA(diameter 8-12mm)were made between descending aorta and main pulmonary artery,then the self-made stent was delivered to PDA by catheter through femoral artery.The animals underwent echocardiography and artery cineangiography examination immediately after the operation.The anatomi-pathologic examinations were made 1-180 days later.Clinically six cases of PDA(diameter 6-11mm)were also closed with self-made stent in the same way.Results:There were no residual shunt in all animals and patients after the procedure as shown on echocardiography and cineangiography.The patients followed up from 15 to 36 months recover satisfactoryly.Conclusion:The successfully experimental and clinical usgae of self-made stent on large PDA and infant demonstrates that the self-made stent is reliable,effective to treat large PDA(diameter 12-20mm).
