摘  要:目的:分析他汀治疗后出现肝酶升高的相关因素及其转归,评价他汀的肝脏安全性。方法:回顾性总结2007~2010年我院收治的63例服他汀后肝酶升高患者的临床资料,选取63例服他汀后肝酶正常的患者作为正常对照。结果:肝酶升高组较正常组应用较大剂量他汀的比例明显增加(P<0.05),前者的转氨酶基线水平较后者升高(P=0,00),而两组的性别、年龄、BMI、LDL-C降幅、总胆红素水平、他汀的种类、合并用药、合并的肝胆疾病未见显著性差异。3个月后肝酶升高组均恢复至正常转氨酶水平,肝酶明显升高时他汀的停药率增加(P<0.05)。结论:他汀治疗后出现肝酶升高,通常是无症状、可逆的,呈剂量依赖性,总体上他汀是安全的,有待于更充分合理的应用。Objective: To investigate the relative factors and prognosis of elevated liver transaminase levels after statin therapy. Methods:We retrospectively analysed the clinical data of 63 cases with elevated liver transaminase values caused by statins in our hospital from 2007 to 2010. Results: The group of elevated transaminase had higher dose of statin and higher baseline of transaminase level than control group(P〈0.05), had no difference in sex, age, BMI, LDL-C reduction, total bilirubin level, type of statin, coadministration and concomitant liver and gallbladder disease with control group and had normal transaminase values after 3 months of discharge. The ration of statin withdrawal in group of marked aminotransferase elevations statistically increased(P〈0.05). Conclusions:The elevation of liver transaminase after statin use is usually asymptomatic, reversible, and dose-related. Overall statin is safe and statin utilization should be more sufficient and reasonable.
