24例跟骨骨折术后应用VSD治疗亚急性期伴皮肤软组织坏死缺损的护理

摘　　要：目的:探讨应用封闭负压引流(VSD)治疗跟骨开放粉碎性骨折术后亚急性期伴皮肤软组织坏死缺损的的效果。方法:2007年1月～2008年6月间,将36例患者随机分为两组,24例作为实验组,采用手术切开复位内固定的同时辅助恒定负压封闭引流技术治疗。12例作为常规治疗组,对比分析两组患者术后伤口愈合、患者满意度、术后并发症、预后及平均住院天数。结果:使用VSD的实验组伤口Ⅰ期愈合及满意度均显著高于常规治疗组,平均住院天数也显著低于常规治疗组。实验组总有效率96%,对照组总有效率86.36%。结论:VSD使治愈率明显提高,积极的心理疏导,治疗前彻底的创面清创,严格的消毒隔离,高负压引流的有效保持,是VSD治疗跟骨开放粉碎性骨折术后亚急性期伴皮肤软组织坏死缺损的重要保证。Objective：To study the effect of the application of vscuum sealing drainage （VSD） to open calcaneal fractures with skin and soft tissure defect of necrosis in middle stage.Methods：Between January 2007 and june 2008,36 cases were randomly divided into two groups,24 cases as the test group,With open reduce internal fixation （orif） and constant the application of vacuum sealing drainage. 12 cases as routine treatment group,comparison and analysis of the two groups：postoperative wound healing,patient satisfaction,postoperative complications,prognosis and the average days in hospital.Results：The use of VSD wound healing experimental I period and satisfaction with were significantly higher than the routine treatment group.The average days in hospital are significantly lower than conventional treatment group.The effectiveness of test group the effectiveness of comparison group is 86.36%.Conclusion：The cure rate with the VSD was improved obviously.Positive psychological persuation, treatment of wounds completely before debridement, strict disinfection and the maintain of negative pressure drainage is the guarantee of vscuum sealing drainage （VSD） to open calcaneal fractures with skin and soft tissure defect of necrosis in middle stage.
