早期肠内营养支持加调节肠道菌群制剂与胃癌术后早期并发症发生的关系探讨

摘　　要：目的探讨胃癌术后早期肠内营养加调节肠道菌群制剂与胃癌术后早期并发症发生的关系。方法回顾性分析我科2008～2010年早期肠内营养支持加调节肠道菌群制剂的胃癌术后患者36例与早期肠内营养未加调节肠道菌群制剂的胃癌术后患者38例发生早期并发症的临床资料。患者的胃癌分期,术前身体状况评估,手术方法,手术时间无明显差异。结果两组患者术后发生腹泻(或腹胀)、感染(腹腔,肺部及其胸腔)、吻合口漏、肠梗阻、全身炎症反应综合征(SIRS)和多器官功能障碍综合症(MODS)等的发生率分别为19.4%和39.4%(P<0.05)、2.5%和3.5%(P>0.05)、2.0%和2.2%(P>0.05)、1.5%和2.8%(P>0.05)、2.2%和11.6%(P<0.05)、0%和0.08%(P<0.00)。结论早期肠内营养支持加调节肠道菌群制剂是减少胃癌患者术后早期并发症的重要措施之一。Objective To investigate the relationship between early postoperative enteral nutrition plus intestinal flora regulator and early postoperative complications of gastric cancer.Methods Retrospective analysis of the clinical data from 2008 to 2010 about 36 cases of postoperative gastric cancer patients,who were cured with early postoperative enteral nutrition plus intestinal flora regulator and those of 38 cases only cured with early postoperative enteral nutrition.Staging of gastric cancer patients,preoperative physical condition assessment,surgical technique and operative time had no significant difference.Results The incidence rates of the two groups on postoperative diarrhea（or abdominal distension）,infections（abdominal,lung and chest cavity）,anastomotic leakage,intestinal obstruction,systemic inflammatory response syndrome（SIRS） and multiple organ dysfunction syndrome（MODS） were 19.4% and 31.7%（P 0.05）,2.5% and 3.5%（P 0.05）,2.0% and 2.2%（P 0.05）,1.5% and 2.8%（P 0.05）,2.2 % and 11.6%（P 0.05）,0% and 0.08%（P 0.00）.Conclusion Early postoperative enteral nutrition plus intestinal flora regulator is one of the important measures to reduce early postoperative complications of gastric cancer.
