阴道彩超监测药物流产终止早早孕的临床研究

摘　　要：目的探讨阴道彩超监测下米非司酮配伍米索前列醇终止早早孕（停经≤42天）的临床效果,以降低药物流产的失败率,寻找合适的药物流产时间。方法对36例门诊既往月经规律,停经42天以内,查尿HCG阳性,阴道彩超探测提示宫内早早孕的健康妇女,自愿接受药物终止妊娠,用药后观察阴道流血时间、量、腹痛情况以及绒毛排出情况。28例当天见小绒毛排出,另8例未见典型绒毛排出。15天后复查尿HCG35例已转为阴性,1例阳性。35例阴道流血已净,1例有少许阴道流血。34例复查阴道彩超子宫内膜恢复正常,1例宫腔积液较多,1例宫内孕囊较前增大,该2例均行清宫术,流产完全率为94.44%,不完全率为2.78%,失败率为2.78%,药流后40天随诊,月经全部正常来潮、无感染、宫腔粘连、月经不调等并发症。结论结合阴道彩超确诊的早早孕妇女此时间用药安全可靠,成功率高,克服了药物流产时间长,流产不全的缺点。Objective To investigate the clinical result of stopping the very early pregnancy（stop menstruate time=42 days） by mifepristone and misoprostol under the monitoring of transvaginal color ultrasound.To looking for a suitable time for medical abortion and redusing the failure rate of the medical abortion.Methods A total of 36 healthy women who came to the out-patient department were included into this study.They were all since go toward menstruation regulation,stopping menstruate in 42 days,checking to wet HCG masculine gender.The pregnanacy of intrauterine was confirmed by the transvaginal color ultrasound.They all voluntarily accepted this medicine to terminate gestation.Observe the bleeding time and quantity,stomachache and floss to eject a circumstance after using a medicine.28 of them had seen the small floss eject on that day,another 8 had never seen typical model floss to eject.15 days later reexamining to the wet HCG,35 examples had already turned into feminine gender,one was masculine gender.35 vaginas bleeding had been clean,one had few vagina bleeding.34 reexamination vagina film instauration inside the colourful super wombs were normal,one example accumulated a liquid more,the embryo inside another one was more exto-enlarge,this 2 all came to a surgical terminate.The completely miscarriage rate leads to 94.44%,the incompletion leads to 2.78%,the failure rate is 2.78%.Following with visit behind 40 days,the menstruation was all normal to come to tide.They have no complications such as infection,temple chamber glue connecting,the menstruation inadjusting etc.Conclusion Combining vagina colourful super confirming an early pregnancy,taking this medicine is safe and credibily at the early time.It has a high success rate.It also can overcome the weakness of having a long miscarriage time and incompletely by a single medical abortion.
