PAI-1在慢性肾脏病肾小球的表达及意义

摘　　要：目的研究肾小球纤溶酶原激活物抑制剂-1(PAI-1)在IgA系膜增生性肾小球肾炎、非IgA系膜增生性肾小球肾炎、膜性肾病、局灶增生硬化性肾小球肾炎、微小病变性肾病患者肾组织的表达情况,探讨其表达水平与肾脏病理变化及临床资料之间的关系。方法采用免疫组化法和计算机图文分析系统对45例入选疾病组和5例正常对照组的PAI-1的表达水平进行检测,并结合临床资料和肾脏病理进行统计学分析。结果正常对照组肾小球PAI-1无或极少量表达于肾小球系膜区。与正常对照组相比,疾病组中PAI-1的表达明显增加,两组比较具有显著统计学意义(P<0.01)。随着肾脏病变的加重,肾小球PAI-1的表达也增加,各亚组之间相比有统计学差异(P<0.05或P<0.01)。肾小球PAI-1的表达与血肌酐正相关(相关系数为0.627);与尿蛋白定量正相关(相关系数为0.871),与患者白蛋白、总胆固醇、游离胆固醇、甘油三酯、高密度脂蛋白、低密度脂蛋白及脂蛋白(a)LP(a)无相关性。结论 (1)肾小球PAI-1在正常肾组织少许表达,在疾病组表达增加;随着肾脏病变加重,肾小球PAI-1的表达也增加。与血肌酐、尿蛋白正相关,PAI-1在肾脏病的发生、发展中起重要作用;(2)PAI-1可作为预后判断的一个可靠指标,也可作为治疗靶点,降低PAI-1的表达可减缓肾脏疾病的慢性进展。Objective To investigate the expression of Plasminogen activator inhibitor-1（PAI-1） in renal tissue of patients with the chronic renal disease including The minimal change disease,IgA mesangial proliferative Glomerulonephritis,mesangial Proliferative Glomerulonephritis.MsPGN：Membranous nephropathy.focal glomerulosclerosis.we also study the relationship between the expression levels of PAI-1 and their clinical manifestation.Methods The expression sites and levels of PAI-1 in 44 renal tissue of patients and 5 cases of normal controled group were exmined by immunohistochemical and computer image analysis system;We also exminated the relationship between the expression of PAI-1 in renal tissue and possible associated clinical datum.Results there is none or few expression of PAI-1 in normal glomeruli.Compared with the normal control group,it was significantly higher than the diseased group（P0.01）.The expression was increased gradually in diseased group with the degree of renal injury,there is statistical different among subgroup each other（P0.05：P0.01）.There was signif icant positive correlation between the expression of PAI-1 in renal tissue with the Serum creatinine（r=0.627）,guantity of proteinuria in 24 hours（r=0.871）.but no correlation was found between the expression of PAI-1 with the albumin,total cholesterol triglyeride,Low-density lipoprotein,High-density lipoprotein,lipoprotein（a）.Conclusion The over expression of PAI-1 may play an important role in contributing to the pathogenesis and development of glomerulonephritis,it can be used estimate the patients consult.PAI-1 may be considered as a potential therapeutic target,the progressive loss of renal function can be improved.
