儿童紫癜性肾炎的危险因素研究

摘　　要：目的分析儿童过敏性紫癜(HSP)的发病率与肾脏损害的危险因素。方法将2008年1月～2010年5月入住河南科技大学第一附属医院的110例HSP患儿,分为紫癜性肾炎(HSPN)组和非肾炎组,对性别、年龄、季节、居住环境、临床表现及实验室检验等因素进行单因素和多因素分析,单因素用2检验,多因素用Logstic回归分析(P<0.05)。结果①HSP中72例出现肾脏损害,发病率为65.5%,94.4%HSPN患儿尿检异常发生在起病1个月内,且在2周内的有63例(87.5%)。②经单变量分析,皮疹持续时间及皮疹3月内反复≥3次、消化道出血、年龄>7岁、腹痛与HSPN发生密切相关(P<0.05)。经逐步Logstic回归分析示,年龄>7岁与HSPN发生密切相关(P<0.05)。结论 HSP患儿中年龄>7岁,易出现肾脏受累,应严密监测并采取积极的治疗措施以防止肾脏损害的发生和进展。Objective To analyze incidence of children Henoch-Schonlein purpura and the correlated factors of renal damage. Methods 110 cases of the first hospital affiliated to Henan University of Science and Technology with HSP from Jan.2008 to May 2010,were divided into purpura nephritis（HSPN） and non-nephritis groups.Such factors as sax,age,onset season,living,clinical and laboratory tests were dealt with by single-and multi-variable analysis.Data were analyzed for statistical significance by means of x2 test and binary Logistic regression（P0.01）. Results ① 72 cases（65.5%） in HSP were with kidney damage,94.4% HSPN children with abnormal urine test within 1 month.And 63 cases（87.5%） HSPN children with abnormal urine test within 2 weeks.② Symptoms of severe rash,skin rash repeated in three months≥3 times,astrointestinal bleeding,older than 7 years,persistent severe abdominalpain were closely related to the occurrence of HSPN.Data were analyzed for statistical significance by means of x2 test with single variable analysis（P0.05）,older than 7 years,Logistic regression analysis had statistical significance（P0.05）. Conclusion In HSP older than 7 years with kidney easily involved,they should be cosely monitored in terms of treatment and positive measures taken to prevent the occurrence of kidney damage and progress.
