宁夏山区三县特需人群口服碘油丸前后碘营养状况的调查

摘　　要：目的调查宁夏山区特需人群(45岁以下已婚育龄妇女、孕妇、哺乳期妇女)口服碘油丸前后碘营养状况,评价碘油丸的补碘效果。方法 2008年4月对宁夏盐池、海原、同心三县按照中国CDC地方病控制中心要求的PPS抽样法抽取444份特需人群尿样,针对特需人群实施碘油丸补碘措施;3个月后特需人群再次抽取服用碘油丸的特需人群尿样300份。采用酸消化砷铈接触法(W S/T107-1999)测定服用碘油丸前后两批尿样尿碘水平。结果盐池、海原、同心三县特需人群服用碘油丸前尿碘中位数(MUI)分别为143.65、148.05、143.68μg.L-1;服用后MUI分别为230.65、227.50、258.45μg.L-1,三县尿碘差异未见统计学意义(P>0.05)。特需人群服用前后MUI分别为143.50和236.00μg.L-1,服用后尿碘水平高于服用前(P<0.05),总体尿碘适宜比例由78.2%增加到82.0%(P<0.05),已婚未孕妇女适宜比例由80.6%降低到69.2%(P<0.05)。结论口服碘油丸可作为特需人群的辅助补碘措施,但不同生理期妇女服用碘油丸后保护效应不同,不同生理时期应选择相应补碘措施。Objective To compare Iodine nutrition level of target population before and after taking iodine-oil capsules in Mountainous Regions of Ningxia and to evaluate the iodine nutrition supplement effects of iodized oil capsules.Methods 444 unine samples were collected using PPS method in mountainous regions of Ningxia（Yanchi County,Haiyuan County and Tongxin County）.Urinary iodine level were determined by the method of Acid digestion of arsenic-Ce contact（WS/T107-1999）.Iodine nutrition were compared before and after taking iodine-oil capsules.Results Before Taking lodized oil capsules,the urine iodine medians of three target populations were 143.65、148.05、143.68μg·L-1 respectively;after taking lodized oil capsules,the urine iodine medians of three populations were 230.65、227.50、258.45μg·L-1 respectively.No significant difference was found in three county urines iodine0.05）.Before and after taking the capsules,their urine iodine medians were 143.50 and 236.00μg·L-1 respectively（P0.05）.After taking capsules,suitable proportion of the overall urinary iodine increased from 78.2% to 82.0%（P0.05） and Suitable proportion of married unpregnant women decreased from 80.6% to 69.2%（P0.05）.Conclusion Oral iodized oil capsules can be used as one measure to support special needs populations iodine nutrition.Different population have to select different level because people at different physiological periods have different protection rates.
