两种加温输液方式对乳腺癌根治术患者的影响

摘　　要：目的探讨不同方式的加温输液对乳腺癌根治术患者术中体温、出血量及术后苏醒的影响。方法选取乳腺癌根治术患者60例,按照随机数字表法分为对照组30例,术中输液经恒温水箱加温至40℃;观察组30例,术中输液经专用电子输血输液加温仪加温至40℃,比较两组患者麻醉前和全身麻醉插管后30 min、60 min、90 min、120 min以及手术结束时的体温、出血量、手术时间、苏醒时间、气管拔管时间、寒战及躁动发生率等指标。结果观察组术中出血量、苏醒时间、拔管时间、寒战和躁动发生率均低于对照组(P<0.01或P<0.05)。结论经专用电子输血输液加温仪输液,能够提供恒温液体,维持患者术中正常体温,减少术中出血量,缩短全身麻醉后苏醒时间和气管插管拔管时间,降低寒战、躁动发生率,值得临床推广使用。Objective To explore the effects of two methods of heated infusion on the body temperature,bleeding volume and postoperative awakening time in patients with breast radical mastectomy. Methods Sixty patients with breast radical masteetomy were randomly divided into the control group （ 30 cases） and the intervention group （30 cases）. All the two groups received transfusion during the operation ,and the fluid was heated up to 40 ~C. The thermostatic water tank was used to heat the fluid in the control group,and the specialized electronic heating apparatus was used in the intervention group. The patients＇body temperature,bleeding volume ,awakening time,extubation time,the incidences of shivering and restlessness were compared between the two groups before the general anesthesia,30 min,60 rain,90 min and 120 min after the intubation,and at the end of the surgery. Results The bleeding volume and the incidences of shivering and restlessness in the intervention group were significantly lower than the control group（ P 〈 0.05）, and the awakening time, extubation time in the intervention group were shorter than the control group （P 〈0.01 ）. Conclusion Using the specialized electronic heating apparatus to heat fluid can reduce the bleeding volume,shorten the awakening time and extuhation time,reduce the incidences of shivering and restlessness. It is worth being applied widely in clinical.
