基于肺肠相关的ARDS研究思路探讨

摘　　要：急性呼吸窘迫综合征(ARDS)的临床表现突出体现了中医"肺肠相关"的特点。"肺病及肠"、"肺肠同病"是从中医视角观察ARDS的常见结论。基于肺肠相关特征的ARDS研究可以从以下三个方面开展工作:1)开展ARDS脏腑证候分布及演变规律的临床流行病学调查,明确其肺肠相关的证候学特征,从而丰富中医"肺与大肠相表里"的理论内涵。2)开展肺肠同治、肺病治肠、肺病治肺方法对ARDS证候演变及相关物质基础影响的临床研究,阐明不同治疗方案产生疗效的物质基础,寻找临床合理治疗方案,将有助于揭示病理状态下肺与大肠相互影响的内在联系和协调机制。3)开展ARDS肺肠相关的炎症调控与黏膜免疫机制的基础研究,将为揭示"肺与大肠相表里"这一脏腑相关理论的现代生物学机制提供实验学证据。Clinical manifestations in Acute respiratory distress syndrome outstandingly reflect the characteris tics of the relevance of Lung and Large Intestine.Lung disease impacting on intestine and meanwhile onset of Lung and Large Intestine are the common conclusion on the standpoint of TCM.Based on the theory,we can research the ARDS from the following three aspects to carry out our work：Carrying out viscera syndromes distribution and evolu tion on ARDS with clinical epidemiologic survey and clarifying characteristics of relevance of Lung and Large Intes tine will enrich the theoretical connotation of ＂Lung and Large Intestine exterior-interior＂.Conducting clinical re search on syndrome evolution and influence of related material with different therapy.Illustrating different treatment effects of material base and looking for rational clinical treatment will help reveal the interplay and inner relation and coordination mechanism between Lung and Large Intestine under the pathologic state.Carrying out basic research on regulating of inflammation and mechanism of mucosal immunity will provide laboratorial evidence for the theory of ＂lung and large intestine exteriorinterior＂ with modern biological mechanisms.
