基层医院呼吸道鲍曼不动杆菌感染情况分析

摘　　要：目的分析基层医院鲍曼不动杆菌导致的下呼吸道感染情况,以加强预防相关感染的发生。方法 回顾2009～2010年我院呼吸道感染患者送检的痰标本,选出鲍曼不动杆菌感染患者,根据送检病区和患者年龄进行统计分析,并对药敏结果进行统计。结果 722例呼吸道感染患者送检样本中28例发现鲍曼不动杆菌。ICU感染率20.9%,普通病区感染率1.6%(P<0.05);以80岁为界分组对比,累计检出率80岁以下组1.8%,80岁以上组5.5%(P<0.05);药敏试验敏感度亚胺培南71.4%,头孢哌酮/舒巴坦64.3%。阿米卡星和哌拉西林/他唑巴坦21.4%、头孢吡肟17.9%、氧氟沙星17.9%、哌拉西林10.7%、头孢噻吩和头孢他定均为7.1%。其余均100%耐药。结论年龄超过80岁和入住ICU发生鲍曼不动杆菌感染的概率明显增高,差别有统计学意义。药敏结果显示鲍曼不动杆菌耐药率高。Objective To analyze respiratory tract infection by Acinetobacter baumannii in montane hospital, to enhance the prevention of associated infections. Methods A retrospective research to the sputum sample from the patients with respiratory tract infections from 2009 to 2010, and count the number of Acinetobacter baumannii infection. Analysis the ward which the patients were in, the age of patients, and the susceptibility results of bacteria. Results 28 cases of Acinetobacter baumannii infection were found in 722 cases of respiratory tract infection. ICU infection rate was 20.9%, general ward infection rate was 1.6%, P0.05. The cumulative detection rate of under 80-year-old group was 1.8% and that of the group of over 80 years was 5.5%, P0.05. Susceptibility testing： imipenem 71.4%; cefoperazone/sulbactam 64.3%. Amikacin and piperacillin/tazobactam 21.4%, cefepime 17.9%, ofloxacin 17.9%, piperacillin 10.7%. Cephalothin and ceftazidime were 7.1%. The others were 100% resistance. Conclusion Age over 80 years old and stay at the ICU are significantly higher probability of Acinetobacter baumannii infection. Susceptibility results show that resistance rates of Acinetobacter baumannii is high.
