摘　　要：上海市青浦区于2009年开始引入并着手实施"户籍医生制"社区卫生服务模式,户籍医生以村庄为责任区,以农民家庭为服务单位,以村卫生室及健康俱乐部为服务平台,在村委干部、健康管理志愿者等的协助下,为农民提供基本医疗和公共卫生服务。实施一年来,农民对户籍医生的知晓度、对慢性病知识的了解、社区卫生服务有效利用状况及总体满意度有了明显提高。The rural community health service mode based on household doctor system was introduced in QingPu District of Shanghai in 2009.Taking the village clinics and health clubs as service platform,rural residents＇ family as service unit,the household doctors provided basic medical and health services for them,which assisted by managers or volunteers.One year later,rural residents＇ awareness of household doctor system and chronic diseases had significantly increased.In addition,utilization of community health services and rural residents＇ satisfaction had also improved.
