摘  要:目的观察阻断肾素-血管紧张素-醛固酮系统不同环节对高血压心肌纤维化的逆转作用。方法对15只26周龄自发性高血压大鼠(SHR)分别采用苯那普利、氯沙坦、安体舒通治疗12周,观察治疗前后大鼠血压、心肌胶原含量及Ⅰ型、Ⅲ型胶原容积分数的变化。另取26、38周龄Wistar大鼠各5只,和26、38周龄未做任何治疗的SHR各5只作为对照。结果SHR-26和SHR-38组的血压、胶原含量及Ⅰ型、Ⅲ型胶原容积分数均高于同龄Wistar大鼠组(P<0.05)。苯那普利、氯沙坦治疗后,血压胶原含量及Ⅰ型、Ⅲ型胶原容积分数均较未治疗的SHR-38组明显下降(P<0.05)。安体舒通治疗组血压无明显下降,而胶原含量及Ⅰ型、Ⅲ型胶原容积分数仍明显降低(P<0.05)。结论ACEI、AgII-AT1阻断剂能很好降低血压,减轻心肌间质的胶原堆积。而安体舒通虽未能很好降压,但也能减轻心肌胶原的堆积。Purpose To observe the effect of blockage of the renin angiotensin aldosterone system at different sites on the regression of left ventricular fibrosis in spontaneously hypertensive rats (SHR). Methods Fifteen SHR at 26 weeks were treated for 12 weeks by benazapril,losartan or spironolactone.Myocardial collagen content and the collagen volume fraction(CVF) both of type Ⅰ and type Ⅲ was measured before and after the treatment.Ten Wistar rats (5 at 26 weeks,5 at 38 weeks) and 10 untreated SHR( 5 at 26 weeks and 5 at 38 weeks) were served as controls. Results Blood pressure,collagen content and CVF were all higher in the SHR than in Wistar rats at 26 weeks and 38 weeks old.Benazapril and losartan treatment could reduce blood pressure,collagen content and CVF of the SHR ( P <0.05),but spironolactone could only reduce collagen content (P <0.05) and CVF ( P <0.05) without the reduction of blood pressure. Conclusions Administration of ACEI and AgIIAT1 antagonist can reduce blood pressure,but spironolactone treatment can not.All of the three drugs we used have the same effects on the fibrosis regression.
