个案护士在多学科整合式治疗管理模式中的作用

摘　　要：目的:探讨多学科整合式治疗管理模式中个案护士护理的选择、培训管理及效果评价。方法:选拔培训一批有资质的护理骨干作为个案护士,以特定工作流程为指导,在管理团队领导下开展工作。个案护士通过评定患者、参加多学科小组会议、健康宣教等职责,最终采用患者及家属满意度调查表对个案护士进行评定。结论:个案护士在多学科团队中起到了重要作用,促进了患者康复,减轻了患者心理负担,提高了患者和家属满意率。Objective： The aim of this study was to explore the practical value of how to select and cultivate the case management nurses, also the effect were evaluated with the multidisciplinary integrated treatment management model. Methods： The case management nurses were selected from different departments, where they worked as the backbone of the department. The guideline for case management nurses was the specific work circuit, and they worked under the leadership of management team. The daily work of case management nurses were assessing the patients＇ condition, attending the multidisciplinary integrated treatment management conference and conducting the health education to the patients. Finally, the case management nurses＇ performances were evaluated by the patients and their relatives＇ satisfaction for hospital. Conclusion： The case management nurses played an important role in the team work of multidisciplinary integrated treatment management, their work promoted the patients＇ rehabilitation, decreased the patients＇ mental burden and enhanced the patients and their relatives＇ satisfaction for hospital.
