摘  要:目的:研究血纤维蛋白原及尿酸、白蛋白等指标与冠心病及其病变严重程度的关系。方法:将750例行冠状动脉造影的患者按造影结果分为冠心病组和对照组,用Gensini评分系统评价冠状动脉损害严重程度。采用多因素分析方法探讨冠心病及Gensini积分与血蛋白、尿酸、纤维蛋白原等因素的相关性。结果:冠心病组血白蛋白要显著低于正常对照组,血尿酸、纤维蛋白原要显著高于正常对照组;冠心病危险因素Logistic回归分析显示尿酸、纤维蛋白原等与冠心病独立相关;冠脉Gensini评分危险因素的多元逐步回归分析发现仅高血压史、纤维蛋白原与冠脉Gensini积分独立相关。结论:纤维蛋白原是冠心病的独立危险因素,也与冠心病严重程度独立相关。对于冠心病患者,应监测其纤维蛋白原、尿酸以及血清白蛋白含量。Objective:To understand the relationship between biochemical indicator levels such as blood uric acid,albumin,plasma fibrinogen levels and the severity of coronary heart disease(CHD).Methods: 750 patients were examined by cronary artery angiography and were divided into CHD group and control group.The severity of coronary artery stenosis was evaluated and scored with Gensini coronary scoring system.multi-factors analysis methods were adopted to analyze the relationship between Gensini scoring and clinical-biochemical indicators.Results: The blood albumin of CHD was significantly lower than that of control group;The blood uric acid and fibrinogen of CHD was significantly higher than that of control group.Using Logistic regression analysis to evaluation the risk factor of CHD,the blood uric acid and fibrinogen were introduced equation.Using stepwise multiple regression analysis to evaluation the risk factor of Gensini scoring,the history of hypertension and fibrinogen were introduced equation.Conclusion: The fibrinogen is an independent risk factor for the severity of CHD.The patients with CHD should check their blood albumin,uric acid and fibrinogen levels.
