围绝经期女性发生冠心病的危险因素及相关分析

摘　　要：目的:探讨围绝经期女性发生冠心病的危险因素及相关分析。方法:采用回顾性研究,自2007-11至2010-04在我院心内科因胸闷、胸痛、心悸等症状住院的围绝经期女性患者276例,均行冠状动脉造影、心电图、超声心动图检查。根据上述诊断结果将患者分为两组,冠心病组135例,对照组141例,对两组患者分别进行各项生化指标及女性性激素水平的检测,采用多元Logistic回归分析与围绝经期女性发生冠心病的相关危险因素。结果:冠心病组比对照组的吸烟率、体重指数、总胆固醇、低密度脂蛋白胆固醇、空腹胰岛素、稳态模型胰岛素抵抗指数升高,雌二醇水平降低,差异均有统计学意义(P<0.05);经多元Logistic回归分析表明围绝经期女性冠心病患者与吸烟、体重指数、雌二醇、总胆固醇、低密度脂蛋白胆固醇、空腹胰岛素、稳态模型胰岛素抵抗指数相关。结论:围绝经期女性冠心病发生的主要危险因素与吸烟、体重指数、总胆固醇、胰岛素抵抗、雌二醇、低密度脂蛋白胆固醇有关。Objective ：To analyze the risk factors for coronary artery disease （CAD） in female patients at peri-menopause. Methods：We retrospectively studied 276 female patients at peri-menopause who were treated in our hospital from November 2007 to April 2010 with the major symptoms of chest pain and palpitation. The patients were diagnosed by coronary angiography, and then divided into two groups as CAD group,n = 135 and Control group,n = 141. Various biochemical index and the level of female sex hormones were detected in both groups. Multi regression analysis was performed to analyze the CAD related risk factors in all patients. Results ：The patients in CAD group had more smokers and higher levels of BMI, TC, LDL-C, fasting insulin （HNS） and homeostasis model assessment of insulin resistance （HOMA-IR） than that in Control group, P 〈 0. 05 respectively. The level of estradiol（ E2 ） in CAD group was lower than that in Control group, P 〈 0.05. Multi regression analysis presented that CAD was related to smoking, BMI, E2, TC, LDL-C, FINS and HOMA-IR in female patients at peri-menopause. Conclusion：The major risk factors for CAD in female patients at peri-menopause are related to smoking, BMI, TC, LDL-C, IR and E2.
