强化教育与定期访问对居家老年糖尿病患者血糖的影响

摘　　要：目的:探讨强化教育与定期访问对居家老年糖尿病患者血糖的影响。方法:将60例出院后老年糖尿病患者随机分为干预组和对照组各30例。干预组出院后给予8课时集中强化培训,以后每周电话随访1次,每月家庭访视1次;对照组不给予特殊干预措施。结果:两组干预前空腹血糖(FBG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbA1C)比较差异无统计学意义,4个月后干预组各项生化指标较干预前及同期对照组患者指标显著下降(P<0.01),而对照组各项生化指标较前无明显变化。结论:以护士为主的强化教育与定期电话访问和家访相结合对居家老年糖尿病患者干预效果满意。Objective ： To explore the impact of intensive education and regular visit on the blood glucose level of the elderly diabetic patients at home. Methods： 60 elderly diabetic patients were randomly divided into intervention and control groups （30 cases for each group） after they were discharged from hospital. The patients in the intervention group were given 8 hours concentrated intensive training after their discharge from hospital and then counseling was conducted by telephone every week and home visit was paid each month; the patients in the control group did not receive any special interventions. Results： There were no statistically significant differences between the two groups in the comparison of fasting blood glucose （ FBG）, postprandial blood glucose （PBG） 2 hours after meal and glycosylated hemoglobin （HbA1 C） before intervention; each biochemical indicator was markedly reduced after intervention for 4 month compared with that before intervention in the intervention group and compared with the control group at the same period （ P 〈 0.01 ） ; the change of each biochemical indicator was not much significant than ever before in the control group. Conclusion ： The effect of nurses based intensive education, regular telephone counseling and home visit is satisfactory in the intervention on the blood glucose level of elderly diabetic patients at home.
