盐水加温冲洗法在无肝素透析中的应用效果观察

摘　　要：目的:探讨在无肝素透析时采用盐水加温冲洗法以改善具有高危出血倾向的血液透析患者体外血路凝血堵管情况的效果。方法:将60例有高危出血倾向的血液透析治疗患者随机分为实验组和对照组各30例,采用无肝素透析,在透析过程中对照组予常规生理盐水冲洗法,实验组予加温盐水冲洗法,观察两组透析前后动静脉端凝血功能化验指标、透析完成情况及透析器管路的凝血情况。结果:实验组相比对照组透后动静脉端凝血风险降低,透析完成率提高,透析器及管路凝血情况明显改善。结论:血液透析患者有高危出血倾向时应用盐水加温冲洗法无肝素透析可改善透析器及管路的凝血情况,达到安全、有效的透析治疗目的。Objective： To evaluate the effect of irrigation with normal saline plus warm water on improving the clotting blockage of extracorporeal blood circuit in the hemodialysis without heparin in the patients with high risk of bleeding tendency. Methods ：60 patients with hemodialysis and with high risk of bleeding tendency were randomly divided into experimental and control groups （30 cases for each group ）. All the patients were given hemodialysis without heparin in both groups. The irrigation with only normal saline was used when the patients received hemodialysis in the control group and the irrigation with normal saline plus warm water was applied in the experimental group. The indicators of coagulation test at the end of artery and vein before and after hemodialysis, the completion of hemodialysis and the clotting in the pipeline of dialyzer were observed in both groups. Results ： The risk of clotting at the end of artery and vein was less after hemodialysis and the completion rate of dialysis was higher in the experimental group than that in the control group according to the results of laboratory tests and the clotting in the dialyzer and pipeline was evidently relieved. Conclusion： The irrigation with normal saline plus warm water can markedly reduce the clotting in the dialyzer and pipeline when it is applied to hemodialysis patients with high - risk bleeding tendency, which can achieve the safe and effective treatment outcome.
