摘  要:目的探讨血铅和血镉及多种危险因素与原发性高血压之间的关系。方法随机抽取杭州城区与农村社区各1个,对40岁以上长住户籍共471人进行问卷调查与健康体检,检测血铅、血镉及其他多种危险因素指标,进行多因素相关分析。结果 (1)共453人获得完整资料,高血压组212人,正常血压组241人,高血压组的年龄、收缩压、舒张压水平均明显高于血压正常组,差异有统计学意义(t值分别为4.03、16.56、10.61,P<0.01)。(2)有高血压家族史、吸烟、饮酒、高盐饮食、高脂饮食和缺乏运动的人数以及体质指数、腰围、总胆固醇和血铅、血镉检测值高血压组明显高于血压正常组,差异有统计学意义(P<0.01),高血压组空腹血糖明显高于血压正常组,差异有统计学意义(P<0.05)。(3)多因素Logistic回归分析显示年龄、高血压家族史、饮酒、高盐饮食、体质指数、腰围、血铅、血镉和总胆固醇与高血压发病有密切关系(OR=1.01~12.77,P<0.01)。结论血铅、血镉和年龄、高血压家族史、饮酒、高盐饮食、肥胖以及总胆固醇增高均与原发性高血压发病有密切关系,在高血压的发生发展中起着一定作用。Objective To investigate the relationship between blood lead,blood cadmium,and other major risk factors and primary hypertension.Methods One urban community and one rural community in Hangzhou were randomly selected for this study.Questionnaire-based survey and physical examination were performed for 471 40-year-old-plus permanent residents from these two communities.Blood lead,blood cadmium,and other major risk factors were measured for multivariate correlation analysis.Results Complete data were obtained from 453 subjects(n=212 in hypertensive group and n=241 in normotensive group).Subjects in hypertensive group had significantly higher age and systolic/diastolic pressure than those in normotensive group(t=4.03,16.56,and 10.61,respectively,P0.01).The number of subjects with a family history of hypertension,smoking,drinking,high-salt/high-fat diets,and lack of physical activity as well as BMI,waist circumference,total cholesterol(TC),blood lead,and blood cadmium were significantly higher in hypertensive group than in normotensive group(all P0.01).The hypertensive group also had significantly higher fasting blood glucose than normotensive group(P0.05).Multivariate logistic regression analysis showed that age,family history of hypertension,drinking,high-sallt diet,BMI,waist circumstance,blood lead,blood cadmium,and TC were significantly correlated with the occurrence of hypertension(OR=1.01~12.77,P0.01).Conclusion Blood lead,blood cadmium,age,family history of hypertension,drinking,high-salt diet,obesity,high TC level are closely related with primary hypertension,and play important roles in the occurence and development of hypertension.
