摘  要:目的观察145例口服葡萄糖-胰岛素释放试验(OG IRT)受试者的β细胞功能和胰岛素敏感性。方法分析145例受试者OG IRT结果,比较正常糖耐量组(NGT组)、葡萄糖调节异常组(IGR组)、2型糖尿病组(T2DM组)受试者的胰岛β细胞功能〔采用胰岛素分泌指数(△I30/△G30)和Homa胰岛素分泌指数(HOMA-β)评价〕及胰岛素敏感性〔采用Homa胰岛素抵抗指数(HOMA-IR)和胰岛素敏感指数(IAI)评价〕。结果 NGT组、IGR组、T2DM组FPG水平依次升高,ΔI30/ΔG30依次降低,组间比较差异均有统计学意义(P<0.01)。IGR组FINS显著高于NGT组(P<0.05)。NGT组、IGR组的HOMA-β显著高于T2DM组,组间比较差异均有统计学意义(P<0.01);IGR组、T2DM组的HOMA-IR显著高于NGT组,IAI显著低于NGT组,组间比较差异均有统计学意义(P均<0.01)。结论从NGT到IGR,胰岛素敏感性显著下降;从IGR到T2DM,胰岛β细胞功能显著受损。Objective To investigate insulin sensitivity and insulin secretion in three groups of normal glucose tolerance(NGT),impaired glucose regulation(IGT) and type 2 diabetes(T2DM) in OGIRT.Methods 73 subjects in NGT group,24 subjects in IGT group and 48 subjects in T2DM group were observed.Insulin secretion was evaluated by △I30/△G30 and HOMA-β.Insulin sensitivity was evaluated by HOMA-IR and IAI.Results Compared with NGT groups,FINS was significantly increased in IGR group(P0.05).Three groups from NGT,IGR to T2DM,FPG level increased continuously,while △I30/△G30 level went down continuously(2.23±0.95,1.63±0.97 and 0.16±1.13 respectively,all P0.01).Compared with T2DM group,HOMA-β was significantly increased in NGT and IGR group(P0.01).Compared with NGT group,HOMA-IR was significantly increased in IGR and T2DM group,and IAI was significantly decreased in IGR and T2DM group(all P0.01).Conclusion The transition from NGT to IGR,insulin resistance level rises continuously,and from IGR to T2DM,β-cell function goes down gradually.
