心血管手术麻醉期间严重急性变态反应临床观察

摘　　要：目的研究探讨心血管手术麻醉期间的严重急性变态反应的临床表现、治疗特点及疗效观察等。方法通过观察300例心血管手术麻醉患者中严重急性变态反应的发生情况,分析并研究它的发生原因和临床表现等。结果在300例患者中,其中有8例出现严重的急性变态反应(其发生率是1.5%);4例可疑变应原是鱼精蛋白,2例进口抑肽酶,其他是2例。8例患者主要表现为严重的低血压,6例患者有支气管哮喘,1例患者出现室颤,3例患者出现皮肤的改变,进行转流抢救者有2例,其中有1例死亡。结论临床上严重的急性变态反应容易在心血管手术麻醉期间发生,但是由于其临床表现相对来说比较隐蔽,手术过程中我们应注意早期及时发现并治疗抢救以免加重病情。Objective Study of cardiovascular surgery of severe acute allergic reactions during anesthesia clinical manifestations,treatment an d therapeutic characteristics such as.Methods By observing the 300 cases of serious cardiovascular anesthesia in patients with the incidence of acute allergic reactions,analyze and study its causes and clinical manifestations.Results In 300 patients,including 8 cases of severe acute hypersensitivity（incidence 1.5%）;4 cases originally suspected allergen protamine,and 2 cases of imports of aprotinin,the other is 2 cases.In these,8 patients were all mainly for severe low blood pressure,bronchial asthma,6 patients,1 patient developed ventricular fibrillation,3 patients had skin changes,to rescue those who bypass 2 cases,of which 1 died.Conclusion Clinically severe acute allergic reaction easily occurred during anesthesia in cardiovascular surgery,but because of its relatively subtle clinical manifestations,surgical process,we should pay attention to the rescue of early and timely detection and treatment so as not to aggravate the disease.
