VIEWS磁共振技术在乳腺癌保乳手术中的应用初步研究

摘　　要：目的:探讨水激励容积成像(Volume imaging with enhanced water signal,VIEWS)磁共振动态增强技术在乳腺癌保乳手术中的价值。方法:对38例乳腺癌的术前钼靶、超声、MRI检查结果与手术病理对照分析。结果:38例乳腺癌磁共振术前诊断均为乳腺癌,其中多中心病灶12例;钼靶术前发现乳腺肿块17例;超声术前发现乳腺肿块37例,超声报告多中心病灶3例,钼靶未报告多中心病灶。本组保乳手术18例,首次切缘阴性率为95%(17/18)。结论:VIEWS动态增强磁共振成像对乳腺癌的诊断、多中心乳腺癌的显示及乳腺导管内浸润的判断均有明显优势,对于明确乳腺癌保乳适应症以及准确把握手术的切除范围具有较高的临床应用价值。Objective：To evaluate VIEWS dynamic enhanced MRI for breast-conserving surgery.Methods：38 cases of breast cancer,surgically and pathologically proved,were enrolled in our series.The cases detected by preoperative CR mammography,ultrasonography,MRI were analyzed.Results：Preoperative MRI detected 38 cases of breast cancer,including 12 cases of multicentric lesions.Preoperative CR mammography detected 17 cases of breast masses.Preoperative ultrasounography deteced 37 cases of breast masses,including 3 cases of multicentric lesions.CR mammography did not find any multicentric lesions.The primary negative margin rate was 95%（17/18） in the 18 patients undergoing breast-conserving surgery.Conclusion：VIEWS dynamic enhanced magnetic resonance imaging has the superiority to detect breast cancer with multicentric lesions and extensive intraductal filtration,which suggests it is useful to determine the indication and the extent of surgical excision in breast-conserving surgery.
