摘 要:目的阐明CD71(转铁蛋白受体,TfR)在IgA肾病肾组织中的特征及其与IgA分子表达特征之间的相互关系,探讨其是否为人系膜细胞(HMC)主要的免疫球蛋白A(IgA)受体。方法 60例肾活检患者根据临床表现和肾活检病理诊断分为3组:原发性IgA肾病组(原发组)20例;继发性IgA肾病组(继发组)20例;非IgA系膜增生性肾小球肾炎组(对照组)20例;另择5例正常肾组织为正常对照组(正常组)。肾活检组织常规行光镜、电镜及免疫荧光检查。同时,每例肾活检患者的石蜡切片经充分脱蜡后采用免疫组化SP法观察CD71和IgA在肾组织的表达特征及两者之间的相互关系。结果 CD71在肾组织中肾小管上皮细胞呈阳性表达,在肾小球无表达;CD71在原发组、继发组及对照组中的表达无显著差异(P>0.05);IgA在原发组和继发组患者的肾小球上呈现不同程度的阳性表达;CD71与IgA分子非共位表达,两者表达无相关性。结论 CD71可能并非人系膜细胞主要的IgA受体。Objective To study the expression features of CD71(Transferrin receptor,TfR) in the tissue of IgA nephropathy and correlation between CD71 and IgA,and to investigate whether CD71 is the main immunoglobulin A(IgA) receptor of human mesangial cell.Methods According to clinical manifestations and pathological diagnosis,renal tissue samples of 60 cases were divided into the primary IgA nephropathy(the primary group,20 cases);the secondary IgA nephropathy(the secondary group,20 cases),non-IgA mesangial proliferative glomerulonephritis(the control group,20 cases);at the same time 5 cases of normal renal tissues were chosen as normal control(the normal group).Renal tissues were conventionally examined by electron microscope,light microscope and immunofluorescence,paraffin sections of every renal biopsy patients were observed by immunohistochemistry after totally deparaffin to illustrate the expression features of CD71 and IgA and the relations between them.Results The expression of CD71 was positive in renal tubular epithelial cell,no expression in glomeruli;there was no significant difference in the primary group,the secondary group and the control group(P0.05).The expression of IgA in glomeruli of the primary group and the secondary group was found with different levels.The expression of CD71 did not co-localized with IgA,there was no association between them.Conclusion CD71 is not the main IgA receptor of human mesangial cell probably.
