连续缝合法进行胰肠吻合在胰十二指肠切除术中的应用

摘　　要：目的 探讨在胰十二指肠切除术中采用连续缝合法进行套入式胰肠吻合对预防胰漏发生的作用.方法 对22例胰十二指肠切除患者采用4-0可吸收线连续缝合法进行端侧套入式胰肠吻合.结果 所有患者均顺利施行套入式连续胰肠吻合,平均胰肠吻合时间13（8～20）min,全部病例均未出现胰肠吻合口漏,无手术死亡;出现胰腺残端出血1例,经保守治疗治愈;术后胆漏1例,经再次手术后治愈. 结论连续缝合法行端侧套入式胰肠吻合适用于任何情况下的残余胰腺,且操作简便、省时、并发症少,是胰肠吻合的一种有效改进.Objective To investigate the effect of prevention of postoperative pancreatic leakage by continuous invaginated suturation of the pancreaticoenterostomy in pancraticoduodenectomy.Method Twenty-two cases underwent pancraticoduodenectomy were performed by continuous invaginated pancreaticoenterostomy with 4-0 absorption lines.Results All cases were performed continuous invaginated pancreaticoenterostomy successfully with the average time of 13（8～20）minutes,the operative time was shortened obviously,and there was no pancreaticoenterostomy leakage in all cases,no death happened.One patient was complicated by pancreatic remnant bleeding and was cured by the conservative therapy.Another patient occured bile leakage and was cured by reoperation.Conclusion Continuous invaginated pancreaticoenterostomy can be applicable to any situation for the residual pancreas with the advantages of easy manipulation,little operative time and few complications,thus it is an effective improvement of the pancreaticoenterostomy for the prevention of the pancreatic leakage.
