摘　　要：目的 探讨单纯改良后路切口后柱重建板前柱空心加压钉内固定治疗髋臼双柱骨折的临床疗效.方法 对12例髋臼双柱骨折患者,均采用单纯改良后路切口后柱重建板前柱空心加压钉内固定治疗.结果 12例平均随访18（10～36）个月.骨折复位和关节功能按Matta标准进行评价：解剖复位10例,满意复位2例;关节功能优10例,良2例.术前合并坐骨神经损伤的2例均完全恢复,未出现股骨头坏死、创伤性关节炎;3例出现轻微异位骨化. 结论单纯改良后路切口后柱重建板前柱空心加压钉内固定治疗髋臼双柱骨折疗效优良,创伤较联合切口小.Objective To explore the clinical effect of internal fixation of reconstructive plate and hollow compressive screws in the treatment of acetabular both-column fractures via single modified Kocher-Langenbeck approach.Method 12 cases with acetabular both-column fractures were treated by internal fixation via single modified Kocher-Langenbeck approach,including reconstructive titanium plate fixation of posterior column and hollow compressive screws fixation of anterior column.Results All 12 cases were followed for 10 to 36 months with an average of 18 months.According to Matta＇s score standard,anatomical reduction was obtained in 10 cases while satisfactory reduction in 2 cases,and the joint function revealed excellence in 10 cases and good in 2 cases.The sciatic nerve injury was complete recovery in 2 cases without femur head necrosis and traumatic arthritis.Slight heterotopic ossification was found in 3 cases.Conclusion The internal fixation of reconstructive plate and hollow compressive screws for acetabular both-column fractures via single modified Kocher-Langenbeck approach can achieve excellent effect with minimally trauma.
