摘  要:目的 探讨16层螺旋CT对先天性脊柱侧弯的诊断价值.方法 收集自2007-06-2010-04经放射学检查和临床诊治的先天性脊柱侧弯患者40例.术前均行16层螺旋CT扫描,然后在工作站完成多平面和曲面重组(MPR/CPR)、表面最大密度投影(SurMIP)和容积重建(VR),评价不同后处理图像的应用价值.结果 VR可以清晰评价先天性脊柱侧弯的所有相关表现,尤其适用于脊柱侧弯的分类.MPR/CPR重建图像能清晰显示椎弓根层面,显示畸形椎体、椎管并进行椎体旋转角度测量.40例先天性脊柱侧弯中,脊柱侧弯越重则顶椎的旋转度数越大,各椎体椎弓根矢状径均大于横径且凹侧椎弓根横径均小于凸侧,相邻椎体有类似表现.SurMIP可以用来测量脊柱侧弯角度,类似于X线片.结论 MSCT的后处理技术对矫形手术方法的制定有重要价值.Objective To discuss the value of 16--slice spiral CT for diagnosis of congenital scoliosis. Methods 40 eases of congenital scoliosis during J une 2007 to April 2010 were reviewed. All patients underwent 16--slice spiral CT scan before surgical opera- tionjand images were reconstructed by VR, SurMIP, MPR/CPR. The value of the various images for diagnosis was assessed. Re- suits VR images could directly and clearly show the scoliosis,especially suitable for classification of congenital scoliosis. MPR/CPR images could display clearly the pedicle plane,the vertebrae, spinal canal and the rotation angles of vertebrae could be measured on it. In 40 cases congenital scoliosis, the degree of scoliosis was heavier, the rotation angle of top vertebra was larger. The sagittal diameters of various pedicle of vertebral arch were larger than that of the transverse diameters, and the transverse diameters of pedicle of top vertebral arch in concavity side were short compared of prominence side, the neighboring vertebrae had the similar performance. SurMIP could be used for measuring the Cobb's angle,just as the X--ray film. Conclusion CT post--processing techniques can of- fer better visualization and understanding of the deformities of congenital scoliosis, and play very important role in the orthopaedic surgery of congenital scoliosis.
