网络监测病毒性肝炎病例分类报告干预效果评价

摘　　要：目的提高网络监测病毒性肝炎病例分类报告准确率。方法对杨浦区医疗机构病毒性肝炎病例分类报告实施综合干预措施,对网络监测的病毒性肝炎病例654例(干预前310例,干预后344例)进行流行病学核实,对比干预前后临床分类报告与流行病学核实的一致率,评价干预措施的效果并分析其影响因素。结果甲、乙、丙、戊及未定型肝炎临床分类报告与流行病学核实一致率,干预前分别为91.18%、56.76%、50.00%、94.68%及54.00%;干预后分别为100.00%、66.36%、60.00%、100.00%及89.55%。经χ2检验,干预前后戊、未定型肝炎流行病学核实与临床分类报告一致率差异具有统计学意义(P<0.05);甲、乙、丙型肝炎一致率差异无统计学意义(P>0.05)。结论通过干预,杨浦区病毒性肝炎分类报告准确率得到不同程度提高,其中甲、戊肝分类报告准确率达到了预期效果;而乙、丙肝分类报告准确率虽得到一定程度的提高,但尚需进一步加强。Objective To improve the accuracy of classification of virus hepatitis inspected by network.Methods The intervention was used to improve the accuracy of classification of virus hepatitis inspected by network.And the epidemiologic method was used to check virus hepatitis inspected by network.The contrastive mean was used to analyze the accordance of epidemiological and clinic diagnosis of virus hepatitis before and after the intervention.Results Before the intervention,the accordance rate of epidemiological and clinic diagnosis of virus hepatitis A,B,C,E and unspecific hepatitis was 91.18%,56.76%,50.00%,94.68% and 54.00% respectively;after the intervention,in which was respectively 100.00%,66.36%,60.00%,100.00% and 89.55%.Tested by χ2,the accordance rate of epidemiologily diagnose and clinic diagnose of virus hepatitis E and unspecific hepatitis was significantly rised（P0.05）,virus hepatitis A,B,C was rised in different degree,but there was no statistical significance（P0.05）.Conclusions After the intervention,the accuracy of classification of virus hepatitis in Yangpu District was rised in different degree,in that of virus hepatitis A and E has achieved intervention effect.But in that of virus hepatitis B and C was still to be enhanced.
