参麦注射液治疗老年心力衰竭疗效观察

摘　　要：目的：探讨参麦注射液治疗老年心力衰竭的临床疗效。方法：４０例住院老年心力衰竭患者，在卧床休息、限盐、使用强心剂和利尿剂等综合治疗基础上，随机分为治疗组（加用参麦注射液４０～６０ｍｌ＋５％葡萄糖２５０ｍｌ静滴，每日１次）与对照组（加口服卡托普利１２．５～２５．０ｍｇ，每日３次）各２０例，以１０日为１个疗程。观察２组治疗对心功能、症状、体征的影响和不良反应。结果：第１疗程后治疗组总有效率７０．０％，第２疗程后为９４．４％；而对照组分别为４５．０％与５８．８％（Ｐ＜０．０５和Ｐ＜０．０１）；第２疗程治疗组心率、心率与血压乘积指数较治疗前降低〔分别由（８４．５０±１０．５０）次／ｍｉｎ和１２１．８０±１８．２０降到（７０．７６±９．２０）次／ｍｉｎ和９５．５１±１０．２０，Ｐ＜０．０５和Ｐ＜０．０１〕，对照组略有下降（Ｐ均＞０．０５）；治疗组未见不良反应。结论：参麦注射液治疗老年心力衰竭安全有效，并随着疗程延长可进一步改善心功能。Objective:To investigate the clinically curative effects of Shenmai Injection(参脉注射液) on senile heart failure.Methods:The senile patients with heart failure who were admitted into hospital based on general therapy,including rest on bed,restricting salt,using inotropic and diuretic drugs were randomly divided into treatment group n =20,using Shenmai Injection(参麦注射液) 4060 ml+5% glucose 250 ml infused intravenously once a day and control group ( n =20,using captopril 12 525 0 mg orally three times per day).The therapeutic course was 10 days.The therapeutic influences on cardiac functions,symptoms,signs and harmful reactions were observed in two groups.Results:After one therapeutic course the total effective rate was 70 0% and after two therapeutic course it was 94 4% in treatment group but 45 0% and 58 8% respectively in control group ( P <0 05 and P <0 01).After two therapeutic course in treatment group the heart rate and the product of heart rate times contractibility were significantly depressed than those before therapy (84 50 ±10 50)beats/min vs.(70 76±9 20)beats/min and 121 80±18 20 vs.95 51±10 20 respectively, P <0 05 and P <0 01 ,nevertheless in control group there were only slightly decreased (all P >0 05).No harmful reactions were found in treatment group.Conclusions:In treating senile heart failure Shenmai Injection is safe and effective and as the prolongation of therapeutic course is able to improve cardiac functions further and to achieve the goal of treatment for both the principal and the secondary aspects of disease at the same time.
