摘 要:目的通过侧方液压脑损伤骨折模型研究脑损伤后大鼠血清中降钙素基因相关肽(caleitoningene-related peptide,CGRP)的变化及对骨折愈合的影响。方法 SD大鼠随机分成三组,空白对照组10只(A组,n=10),单纯骨折组48只(B组,n=48),脑损伤骨折组48只(C组,n=48),通过侧方液压脑损伤及开放骨折建立模型,损伤后适时取SD大鼠血清及骨折标本,通过放射免疫荧光、免疫组化、X片、骨折直径及生物特性测量骨痂直径、骨痂中阳性细胞的数量表达及抗应力的大小等,了解血清中CGRP的含量、骨折愈合情况。结果伤后各组血清中CGRP的含量:从伤后CGRF水平的变化看,脑损伤骨折组CGRP含量明显升高,第7天达高峰(196.73±54.98)pg/ml,随后逐渐下降,单纯骨折组CGRP含量也升高,第5天达高峰(74.70±20.76)pg/ml,随后下降;脑损伤骨折组中CGRP含量明显高于单纯骨折组,且高峰时间长,差异有统计学意义(P<0.05);脑损伤骨折组骨痂无论在影像学、直径大小及骨痂抗应力均大于单纯骨折组,免疫组化发现脑损伤骨折组骨痂中阳性细胞明显增多,差异有统计学意义(P<0.05)。结论脑损伤骨折大鼠骨折愈合快于单纯骨折,推测脑损伤血清中CGRP可促进骨折愈合。Objective To utilize lateral fluid percussion traumatic brain injury combined bone fracture model to study the effect of the caIcitonin gene-reIated peptide(CGRP) on the healing of fracture. Methods Sprague-Dawley rats were randomly divided into three experimental groups: controls (A group ,n = 10), fracture only (B group, n = 48 ), traumatic brain injury combined fracture (C group, n = 48 ), the combined Sprague-Dawley rat model of traumatic brain injury and tibia fracture was established by fluid lateral percussion and opened fracture respectively. The CGRP value at 3^rd hour, 1^st ,3^rd ,5^th ,7^th, 14^th day in the plasma of Sprague-Dawley rat after injury at the same time were continuously measured by radioimmune assay. The cell of callus was measured by immunohistochemistry. Bone fracture sample were analyzed by X-ray to study the healing of bone fracture, the size of callus and to meassure the diameter of the callus and biomechanical testing at the 21^st, 28^th day. Results After injury in all the groups, the concentrations of C GRP were increased, the peak was on the 7^rd day in C group, but on the 5's day in B group, then its level in all groups decreased; The levels of serum CGRP of the C group was much higher than that of the B group, the difference was significant(P 〈0. 05) and its last time was longer in C group; at 21 and 28 days after fracture,X-ray photography suggested that the size of callus in C group was significantly larger and denser than those in the B group. In addition the diameter and the stiffness of callus in C group were greater than those in B group, the difference were signiticant (P 〈 0. 05). CGRP was strongly expressed in osteoblasts and chonderocytes and fibroblast in group C. Conclusion The time of fracture healing of rats with brain injury is shorter than that in rats with the bone fracture only, it could supposed that CGRP in serum could promote the healing of fracture
