体外循环心脏直视手术对HB及HBV携带者肝功能的影响

摘　　要：对接受各种心脏病手术的患者于手术前后进行了肝功能测定，旨在观察体外循环对乙肝（ＨＢ）及乙肝病毒（ＨＢＶ）携带者肝功能的影响。其中肝功能正常组３０例、ＨＢＶ携带组（即慢性ＨＢｓＡｇ阳性及大三阳、小三阳）４０例，ＨＢ组（包括大三阳、小三阳并肝功能异常）１４例。结果显示：①体外循环对ＨＢＶ携带者的肝功能有影响，但不显著（Ｐ＞０．０５）；对其术后胸腔引流量无明显影响。②体外循环术后，ＨＢ患者谷丙转氨酶（ＡＬＴ）及谷草转氨酶（ＡＳＴ）显著升高，且其术后胸腔引流量较肝功正常者明显增加。因此认为，必要的护肝治疗，合理的体外循环管理，对预防ＨＢ患者心脏直视术后发生肝功能衰竭、出血等并发症具有重要意义。Abstract Theliverfunctionindexof46patientswithhepatitisBvirus(HBV)and14patientswithhepatitisB(HB)weredeterminedbeforeandaftercardiacoperationTheywerecomparedwiththeliverfunctionindexof30normalpatientsconcomitantly,toinvestigatetheeffectofcardiopulmonarybypassonliverfunctionofpatientswithhepatitisB(HB) ResultsshowthatthelevelsofALTandASTwerehigherinhepatitisgroupthanthoseinnormalandHBVgroup(P<005)andtheirbloodeffusionvolumepostoperativeincreasedsignificantlyinhepatitisgroup(P<005)ThissuggestthatcardiopulmonarybypasscanaffecttheliverfunctionandbleedingvolumeonpatientswithHBVLiverfunctionprotecttherapiesandrationalmanagementofcardiopulmonarybypassdecreaseareimportanttodecreasepostbypassbleedingandpreventvariouscomplicationssuchasliverfailure Kaywords CardiopulmonarybypassLiverfunctionHepatitisBvirusHepatitis
