单核细胞内Ag85抗原对结核性脑膜炎早期诊断的意义

摘　　要：目的探讨脑脊液(CSF)及外周血(PB)单核细胞中Ag85抗原对结核性脑膜炎(TBM)早期诊断的意义。方法选择98例神经系统感染患者,将其中TBM患者52例作为观察组,非TBM患者46例作为对照组。采用免疫细胞化学方法检测CSF及PB单核细胞中结核杆菌早期分泌抗原Ag85复合物蛋白和结核菌素纯蛋白衍化物(PPD),比较PPD、Ag85对于检测TBM的敏感度及特异度。结果 PPD在CSF及BP单核细胞中的特异度分别为84.78%、82.61%,敏感度分别为30.77%、26.92%,差异均无统计学意义(P>0.05)。Ag85抗原在CSF及PB单核细胞中的特异度分别为93.48%、78.26%,敏感度分别为90.38%、42.31%,差异均有统计学意义(P<0.05)。结论采用免疫细胞化学方法检测单核细胞内Ag85抗原诊断TBM简便易行,且敏感度、特异度均较满意,可作为诊断TBM的有效方法。Objective To explore the clinical significance of Ag85 antigen in mononuclear in CSF and PB for early diagnosis of tubercular meningitis（TBM）.Methods 98 cases with nervous system infection were chosen,in which 52 cases with TBM were taken as the observation group,and 46 cases without TBM as the control group.The tubercular antigens PPD and Ag85 in mononuclear in CSF and PB were detected by immunocytochemistry.The sensitivity and specificity of PPD and Ag85 for TBM were compared.Results The sensitivity of PPD in mononuclear in CSF and PB were 84.78%,82.61% respectively,and the specificity were 30.77%,26.92% respectively,but the differences were of no statistical significance（P 0.05）.The sensitivity of Ag85 in mononuclear in CSF and PB were 93.48%,78.26% respectively,and the specificity were 90.38%,42.31% respectively,and the differences were of statistical significance（P 0.05）.Conclusion Detecing Ag85 antigen in mononuclear to diagnose TBM is simple and easy,with satisfactory sensitivity and specificity,which can be used as a good method to diagnose TBM.
