2型糖尿病痛性神经病变患者配偶焦虑情绪和应对方式

摘　　要：目的研究2型糖尿病痛性神经病变患者配偶的焦虑、抑郁情绪及和应对方式的关系。方法对50例2型糖尿病痛性神经病变患者的配偶进行量表测试,包括焦虑自评量表(SAS)、抑郁自评量表(SDS)、应对方式问卷(CSQ)。结果 2型糖尿病痛性神经病变患者配偶SAS评分(44.2±7.61)分高于常模(37.3±12.6)分,差异有统计学意义(P<0.05);按照SAS评分结果,将患者配偶分为研究组(SAS>50分)和对照组(SAS≤50分),比较两组间应对方式,研究组自责、幻想、退避的应对因子分(分别为0.29±0.09;0.58±0.12;0.69±0.21)高于对照组(分别为0.17±0.04;0.31±0.24;0.36±0.17),解决问题应对因子分(0.43±0.19)低于对照组(0.70±0.16),差异有统计学意义(P<0.05);以2型糖尿病痛性神经病变患者配偶的SAS评分为因变量,以各应对因子为自变量,进行多元线性回归,发现解决问题、幻想、退避的应对方式是影响焦虑情绪的主要因素,P<0.05。结论 2型糖尿病痛性神经病变患者的配偶存在明显的焦虑情绪,且和其不成熟型的应对方式有关。Objective To study the spouses′ anxiety,depression and coping strategies of T2DM patients with painful peripheral neuropathy.Methods For the 50 spouses of T2DM patients with painful peripheral neuropathy enrolled in the study,Zung self-rating depression scale（SDS）,self-rating anxiety scale（SAS）,coping strategies questionnaire（CSQ） were used to screen depression,anxiety and coping strategies respectively.Results The scores of SAS in these participants（44.2±7.61） were significantly higher than those in norm（37.3±12.6,P0.05）.According to the scores of the SAS,the 50 participants were divided into test（SAS50） and control groups（SAS≤50） and the coping strategies were compared between these two groups.The test group got higher scores in the coping strategies of self-blaming,withdrawal and imagination（0.29±0.09,0.58±0.12 and 0.69±0.21 respectively） and lower score in problem-solving coping strategies（0.43±0.19） than the control group（0.17±0.04,0.31±0.24,0.36±0.17 and 0.70±0.16 respectively,P0.05）.Multivariate Logistic regression analysis showed that the withdrawal,imagination and problem-solving coping strategies were important factors that affected the anxiety in spouses of T2DM patients with painful peripheral neuropathy（P0.05）.Conclusion The spouses of T2DM patients with painful peripheral neuropathy have obvious anxious emotion which is significantly related to immature coping strategies.
