阿托伐他汀在心房颤动患者中的应用及护理

摘　　要：目的:探讨阿托伐他汀对阵发性房颤或持续性房颤复律后的预防作用及护理方法。方法:将100例阵发性房颤或持续性房颤复律后患者随机分为治疗组与对照组各50例,两组均给予抗血小板药物、抗心律失常药物等常规治疗,治疗组在常规治疗基础上加用阿托伐他汀10 mg/d,对照组口服安慰剂,服药1年,随访1.5年。结果:治疗组阵发性房颤发作次数减少>50%比例高于对照组(P<0.05),阵发性房颤转为持续性或永久性房颤比例、持续性房颤复律后再发率、发生左房扩大者比例、血栓栓塞发生率均低于对照组(P<0.05);随访期间急性房颤发作患者治疗组使用胺碘酮复律剂量及电复律能量、次数均低于对照组(P<0.05)。结论:阿托伐他汀能有效预防或减少房颤发作,治疗期间护理人员应给予患者健康教育和心理疏导,可改善预后、提高生活质量。Objective： To explore the preventive effect of Atorvastatin and nursing methods in the treatment of patients with paroxysmal or persistent atrial fibrillation after cardioversion.Methods： 100 patients with paroxysmal or persistent atrial fibrillation after cardioversion were randomly divided into treatment group and control group（50 patients for each group）.The patients in both groups were treated with anti-platelet and anti-arrhythmic drugs.The patients in the treatment group were additionally given Atorvastatin 10 mg / d and the patients in the control group were given placebo,the treatment lasted for one year and the follow-up was made for one and a half years.Results： The proportion in the decrease over 50% of frequency of paroxysmal atrial fibrillation was higher in the treatment group than that in the control group（P0.05）;the rate of relapse of persistent AF,atrial enlargement and thromboembolism complication was significantly lower in the treatment group than the control group（P0.05）;the use of cardioversion dose of Amiodarone,the power and frequency of electrical conversion for the acute AF patients was less and lower in the treatment group than the control group in the duration of follow-up（P0.05）.Conclusion： Atorvastatin can prevent or reduce the attack of atrial fibrillation.Health education and psychological counseling given by nurses during treatment can improve the prognosis and quality of life of the patients.
