第二趾甲床扩大术在拇指再造中的应用

摘　　要：目的总结足趾甲床扩大术在第二足趾游离移植再造拇指的临床应用经验。方法用这种方法对22例第二足趾移植再造拇指的患者进行了甲床扩大术。在再造指距甲根皮缘0.5cm处,去除一块矩形皮肤,将"U"形皮瓣向近端柔和推剥并缝合。结果再造的拇指全部成活,术后经过顺利,随访1～4.5年,平均2.5年。趾甲外形较好,取得较满意的临床效果。结论在第二足趾游离移植再造拇指中应用趾甲床扩大术,可改善再造拇指的外形,是一种简单有效的手术方法。Objective To summarize clinical application experience of the toenail-bed lengthening in free second toe transfer in reconstruction of the thumb.Methods 22 patients of reconstruction of the thumb with second toe transfer underwent the toenail-bed lengthening.A skin rectangle was resected at a distance of 0.5cm from the eponychium,with care taken to leave the underlying subcutaneous vascular network intact.Then ＂U＂ shape flap was delicately slid proximally and its ends sutured.Results All the thumb of reconstruction had survived completely and the postoperative course was uneventful.At 1～4.5 years（mean 2.5 years） follow up,the cosmetic appearance of the nail was good,with satisfactory clinical results.Conclusion Using toenail-bed lengthening in free second toe transfer in reconstruction of the thumb,the cosmetic appearance of the reconstruction thumb is improved,and this is a simple and effective method of operation.
