纳米银医用抗菌敷料对包皮环切术后疼痛缓解的疗效观察

摘　　要：目的:疼痛是包皮环切术后常见问题,为了探讨包皮术后疼痛缓解的新方法,我们对纳米银抗菌敷料缓解包皮环切术后疼痛的疗效进行了观察。方法:泌尿外科门诊接受包皮环切手术的患者共60例。所有患者术后均不应用止痛药物或其他止痛方法。将患者随机分为2组,试验组术后使用纳米银医用抗菌敷料(深圳市爱杰特医药科技有限公司研制开发)覆盖伤口,对照组按常规方法术后使用凡士林纱布和无菌纱布敷料。术后第1、2、3、5、7天对患者进行随访并根据改良数字疼痛量表进行疼痛评分。各临床因素对痛觉评分的影响采用多元回归分析方法。结果:两组患者全部完成临床观察,所有患者术后均未应用止痛药物或其他止痛方法。患者平均年龄为(31.13±13.94)岁,两组患者的年龄及体重指数(BMI=体重/身高2)均没有统计学差异。对两组患者疼痛评分的比较发现:第1、2、3、5天,试验组的平均痛觉评分均显著低于对照组(P<0.05)。通过多元回归分析发现:年龄、病种、体重指数与痛觉评分无关,只有使用不同敷料对术后疼痛程度有影响。结论:纳米银医用抗菌敷料可明显缓解包皮环切术后的疼痛。Objective： Postoperative pain is a common problem in male circumcision. We investigated the effect of nanometer silver dressing （ Shenzhen AGT Pharm. Co. Ltd. ） in relieving pain following male circumbision. Methods ： Sixty patients undergoing circumcision in the outpatient department were randomized into an experimental and a control group, the incision covered with nanome- ter silver dressing in the former and with vaseline dressing in the latter. None of the patients received any analgesics or other pain-kill- ing therapies after surgery. The postoperative pain intensity was accessed using the modified numeric pain intensity assessment scale at 1, 2, 3, 5 and 7 days after the operation, and statistical analyses were performed using SPSS 12.0 software. Results ： The patients averaged （31.13 _＋ 13.94） years in age, and had no significant differences in age and body mass index （BMI） between the two groups. At 1, 2, 3 and 5 days, postoperative pain intensity was significantly lower in the experimental than in the control group （P 〉 0.05 ）. Multiple regression analysis revealed that postoperative pain score was not correlated with patients^age, BMI and types .of dis- ease, but with the types of dressing. Conclusion ： Nanometer silver dressing can significantly alleviate postoperative pain of circumci- sion, and is particularly applicable to such moist parts as the perineum, genitals, and urethra.
