摘 要:目的:探讨生长激素(GH)、精氨酸与可乐定激发试验联合在矮小儿童诊断中的临床价值。方法:分析对119例矮小儿童和100例正常儿童均进行GH、精氨酸与可乐定激发试验,采用放射免疫分析分别测定激发试验前和激发试验后30min、60min、90min的GH值,并分析比较。结果:矮小儿童与正常儿童比较,空腹血清GH水平无显著性变化(P>0.05),精氨酸与可乐定激发后血清GH峰值有显著性变化(P<0.01)。激发试验的阳性率精氨酸为9.2%(11/119),可乐定为31.9%(38/119),而精氨酸+可乐定为37.0%(44/119)。结论:GH激发试验对矮小儿童生长激素缺乏症(growth hormone deficiency,GHD)诊断有重要意义,可乐定激发试验的阳性率优于精氨酸激发试验,两者联合使用将提高GHD诊断的准确性。Objective To study for the stimulating effect of arginine and clonidine on pituitary growth hormone(GH) secretion in short stature children.Methods 119 cases of short stature children and 100 cases of normal stature children were underwent arginine and clonidine provocation test.Serum GH levels of before and 30min,60min,90min after arginine and clonidine provocation test were determined with radioimmunoassay and analyzing them.Results There were not much difference between the serum GH levels of the short and normal stature children(P0.05),but there were significant different in the peak value of arginine and clonidine provocation test of the short stature children from the normal stature children(P0.01).The short stature children with a GH peak value of 10 ng/ml or above accounted for 9.2%(11/119) by arginine provocation test,but the clonidine had 31.9%(38/119).The arginine and clonidine together had 37.0%(44/119).Conclusion GH provocation test is important for diagnostic value of growth hormone deficiency(GHD) during childhood.The diagnostic value of clonidine provocation test is better than arginine test.The provocativon test using clonidine combined with arginine will increase the accuracy of diagnosis for the growth hormone deficiency.
