冠心病患者静息心率达标情况及β-受体阻滞剂使用情况的现况调查

摘　　要：目的:了解使用β-受体阻滞剂的冠心病(CHD)患者静息心率(RHR)达标情况。方法:选择717例已服用β-受体阻滞剂的CHD患者的早、中、晚RHR进行现况调查,计算其达标率。将早、中、晚RHR分别分为8组(≤50次/min,51～60次/min,61～70次/min,71～80次/min,81～90次/min,91～100次/min,101～110次/min,>110次/min),了解RHR的集中趋势。结果:冠心病患者早、中、晚RHR达标率分别为:10.2%、8.6%、12.7%,均不足15.0%;RHR均数分别为:(75.21±13.12)次/min(早)、(75.29±13.02)次/min(中)、(75.03±13.03)次/min(晚),RHR中位数分别为:73.26次/min(早)、74.34次/min(中)、71.83次/min(晚);早、中、晚RHR≤70次/min的CHD患者比例分别为:42.7%、38.1%、47.4%,均不足50.0%;β-受体阻滞剂的使用剂量偏低,小剂量β-受体阻滞剂的使用率达81.8%。结论:目前CHD患者RHR达标率低,心率控制不理想,β-受体阻滞剂的使用剂量偏低。Objective：To investigate the current status about resting heart rate（RHR） controlling in coronary heart disease（CHD） patients by usingβ-blockers.Method：From June 2009 to November 2009,717 patients were enrolled from different parts of the tertiary hospitals in Hunan province,all of whom were diagnosed as CHD and treated with β-blockers.RHR of these patients were monitored in three periods of time in a day（morning,noon,night） and successful rates of RHR were calculated.Then,we grouped the patients by the RHR into eight groups（≤50 r/min,51-60 r/min,61-70 r/min,71-80 r/min,81-90 r/min, 91-100 r/min,101-110 r/min, 110 r/min） to analysis the central tendendy of RHR.Result：The successful rates of RHR controlling were 10.2%（morning）,8.6%（noon）,12.7%（night）,all of which were less than 15%.Mean of RHR was：（75.21±13.12） r/min（morning）,（75.29±13.02） r/min（noon）,（75.03±13.03） r/min（night）,respectively.Median of RHR was： 73.26（morning）,74.34（noon）,71.83（night）,respectively;The percentages of RHR ≤70 bpm were only： 42.7%（morning）,38.1%（noon）,47.4%（night）,all of which were less than 50%;The β-blockers were used in a small dosage in CHD patients,and the utilization rate of low-dose β-blockers were 81.8%.Conclusion：Most of patients have not controlled the RHR to the standard,the RHR controlling is not ideal,β-blockers dosage was low and not enough.
